Patient feedback is an essential part of continuous quality improvement.

The HRSA Health Center Program Compliance Manual, Chapter 10 states that QI/QA assessments must:
"Assess patient satisfaction, achievement of project objectives, and include a process for hearing and resolving patient grievances."

Listening to patients, responding to concerns, and resolving grievances are key practices that support quality care and organizational improvement.

A stranger sits in the lobby, opens a laptop, and connects to the guest Wi-Fi. Twenty minutes later, the EHR is down.

This is no longer a hypothetical. The new HIPAA Security Rule treats physical and cyber risk as one conversation, because attackers already do.

After thirteen years without updates, the HIPAA Security Rule is being finalized with a 12- to 24-month transition period. The shift is significant: what was addressable is now mandatory.

What is now required:
✓ Access controls and visitor management for data centers and server locations
✓ Encryption of protected health information
✓ Comprehensive risk analysis tied to documented mitigation activities

The dashboard showed 94 percent.

Leadership was pleased. Depression screening rates had climbed three quarters in a row, and the committee was ready to move on.

Then the medical assistant raised her hand.

"When the screening pops up during rooming, most of us click through it. The patient is still settling in. I do not think we are actually screening anyone. We are closing the alert."

The room went quiet.

The 94 percent was real. The screenings were not.

This is the gap that does not show up in any report. The number was accurate. The workflow was compliant. The audit would have passed. And almost none of the patients were actually being screened.

After 20 years in this work, I have learned that the most dangerous metrics in healthcare are the ones that look like they are working. They create the feeling of progress without the substance of it. Leadership celebrates. Boards approve. And the patients the metric was designed to protect walk out unseen.

The medical assistant did something most organizations are not built to reward. She told the truth about her own workflow.

The leaders who move outcomes are the ones who make it safe for that sentence to be spoken, and who treat it as the most valuable data point in the room.

A green dashboard is not the goal. A patient who was actually screened is.

In healthcare, leaders face split-second decisions daily, from patient safety concerns to operational crises. The pressure to "do it all" can be overwhelming.

1. Hire for capability: Look for people who can make sound decisions independently, not just those who follow orders.

2. Empower your team to execute: Give clear authority along with responsibility. If you hired someone to lead quality improvement, let them lead it.

3. Step back from micromanagement: Your role is strategic oversight, not controlling every detail. Trust the expertise you hired.

The reality: Healthcare doesn't need leaders who make every microdecision. It needs leaders who surround themselves with competent people and trust them to do what they were hired to do.

Delegation is how you scale your impact.
