Today is World Sickle Cell Day. πŸŽ—οΈ

Sickle cell disease (SCD) affects roughly 100,000 people in the U.S., the vast majority of them Black and people of Mediterranean and Middle Eastern descent. It's a condition that touches almost every stage of life, including the menopause journey, yet menopause in women with SCD is one of the most under-researched intersections in women's health.

Here's what we do know, and what every woman+ with SCD deserves to hear from her care team: 🩺

πŸ’œ Menopause can come earlier. Women with SCD may experience early menopause (before 45) or even premature menopause (before 40). One contributing factor: SCD can lower anti-MΓΌllerian hormone (AMH), a sign of decreased ovarian reserve.

🌑️ Symptoms can hide in plain sight. Joint pain, sleep issues, mood changes, fatigue, many of menopause's classic symptoms overlap with SCD symptoms, so they often get masked, dismissed, or misattributed.

πŸ’Š HT isn't always the answer. Standard menopause hormone therapy (MHT) carries a higher risk of blood clots, which is already a concern in SCD. That means treatment plans need to be tailored, and you deserve a provider who knows how.

πŸ—£οΈ You can advocate. Ask your hematologist and your gynecologist to talk to each other. Ask whether some of your symptoms may be related to menopause arriving at a younger age than the norm. Ask about a menopause specialist. Bring research with you.

Itchy skin? Probably allergies. Dry eyes? Must be the screens. Ringing in your ears? Stress, right?

What if it's actually the menopause journey? 🀯

These are some of the lesser-known symptoms that women are routinely told are "something else." But research is increasingly clear: as estrogen fluctuates, it can affect everything from skin and tear production to the auditory system.

The trouble is that most doctors aren't trained to connect these dots. So a woman walks in with itching, and walks out with an antihistamine. Walks in with ringing ears, and walks out with a "manage your stress" note. Walks in with three "random" symptoms, and never gets told they might all be telling the same story. 🩺

For LGBTQIA+ folks on the menopause journey, that hunt can be even tougher. Most providers don't get adequate menopause training to begin with, and one who also has expertise in meeting the unique care needs of the LGBTQIA+ community is even more rare. The kind of opinion piece we've been waiting for. Melinda French Gates just published an op-ed in The New York Times calling for a menopause revolution in this country, and we are here for every word of it.

Here's what you need to know: Nearly 1 in 3 American women over 40 experience serious symptoms during their menopause journey. Sleep disruption. Joint pain. Memory issues. Depression. And yet only about 1 in 4 receive any treatment at all. Fewer than a third of American OB-GYN residency programs even teach about menopause. That is a systemic failure, not a personal one.

French Gates is putting significant new funding behind fixing it, targeting research, medical training, policy change and workplace protections. Your menopause journey deserves that level of investment. You deserve a doctor who listens and a path forward that is actually supported by science.

After 11 years and input from 22,000 experts and patients worldwide, polycystic o***y syndrome (PCOS) has officially been renamed polyendocrine metabolic ovarian syndrome (PMOS). The consensus was published in The Lancet in May of 2026, and endorsed by the Endocrine Society and more than 50 other patient and professional organizations. 🩺

Why does a name change matter? Because language shapes diagnosis, treatment, and stigma. The old name was a misnomer: you don't actually need cysts to have the condition, and the focus on ovaries obscured the fact that this is a complex, multisystem hormonal disorder affecting metabolism, cardiovascular health, mental health, and the skin. Up to 70% of those affected went undiagnosed, and care was often fragmented and delayed.

If you have PMOS and you're on the menopause journey, here's what to know:

πŸ• Menopause may arrive later than average.
🌑️ Hot flashes and night sweats may be fewer and less severe.
βš–οΈ Hormone therapy management can be more complicated.
❀️ Your risk for insulin resistance, hypertension, diabetes, heart disease, stroke, high cholesterol, anxiety, and depression is already elevated, and the menopausal transition raises those risks further.

✨ The good news: many of the habits that help with PMOS also help with menopause symptoms and chronic disease risk, including healthy eating, consistent movement, 7 to 9 hours of quality sleep, stress management, and human connection.

Here's something you won't see in most menopause headlines: research shows that women+ in the gender identity and sexual orientation minority often move through the menopause journey with a more neutral, and often more positive, mindset than their cisgender, heterosexual peers.

That doesn't erase the real challenges, from finding affirming providers to navigating health disparities. But it does mean the "menopause is all decline" narrative isn't the only story, and there's a lot the rest of us can learn from those who may have a different perspective than the one society commonly messages.

This Pride Month, we're celebrating the strengths the LGBTQIA+ community brings to this life stage: more positive body image, stronger sexual well-being, and years of practice advocating for the care you deserve.
