District 4 Home Visiting provides voluntary support for high-risk moms and babies during pregnancy and through baby’s first year of life.

Through regular check-ins, education, and care coordination, our team helps identify concerns early and connects families to important resources like healthcare, food, transportation, and infant support services.

By supporting families between medical visits, we help promote healthier pregnancies, healthier babies, and healthier communities.
