The hamstrings are active at various stages of the gait cycle, particularly during knee flexion and hip extension.
The hamstring muscles share a common origin on the lateral aspect of the ischial tuberosity. The semitendinosus and long head of the biceps femoris share a conjoined tendon originating from the lateral facet, while the origin of the semimembranosus lies deeper.
Pathology of the hamstring tendons can occur at various locations. The semimembranosus is one of the most commonly affected components of the hamstring complex in proximal hamstring tendinopathy.

Mennell’s Four Basic Truisms
Mennell’s (1964) truisms provide a theoretical basis for the following hypothesis: Attempting to strengthen muscles when joint motion restriction is present will provide less-than-optimal results, and limited joint ROM needs to be considered during any exercise application.
1. When a joint is not free to move, the muscles that move it cannot be free to move it.
2. Muscles cannot be restored to normal if the joints that they move are not free to move.
3. Normal muscle function is dependent on normal joint movement.
4. Impaired muscle function perpetuates and may cause deterioration in abnormal joints.

The scapulothoracic articulation allows shoulder movement beyond the 120 degrees of elevation provided by the glenohumeral joint. For every 2 degrees of glenohumeral elevation, there is 1 degree of scapulothoracic elevation (McClure et al., 2006; Timmons et al., 2012). The scapulothoracic joint also plays an important role in providing motion and shoulder girdle stability through the 17 muscles that attach to the scapula (Bakhsh & Nicandri, 2018). When these muscles function properly, they provide a stable base for the humerus to glide on and allow for an efficient transfer of force from the lower extremities and trunk. This is accomplished through force-couples of the upper, middle, and lower trapezius as well as the serratus anterior.

Current theory supports the idea that repetitive movements and long periods of poor pos­ture lead to tissue trauma and inflammation. Inflammation, in turn, activates the body’s pain response, which initiates a protective mechanism, increasing muscle tension and causing muscle spasm. These muscle spasms are not like a calf cramp. Heightened activity of the central nervous system (CNS) and tissue mechanoreceptors and nociceptors (e.g., pain receptors) in the injured area create a type of microspasm or tension (Jafri, 2014). As a result of the spasm, adhesions may begin to form within the myofascial tissues. These adhesions form a weak inelastic matrix that decreases normal mobility of the soft tissue (Figure 3-4) (Clark et al., 2010; Money, 2017).

The resting length of a muscle is its state when the body is standing still and the muscle is neither actively contracting nor being stretched.
If the resting length of a muscle is shorter than optimal, the amount of available actin–myosin cross-bridging becomes limited, reducing the muscle’s ability to generate force. Muscles with resting lengths that are longer than optimal will also have limited force-generation capabilities because the overlap between actin and myosin is no longer optimal.
The resting length of a muscle and its ability to contract (shorten) and relax (lengthen) are a consequence of the neural drive it is receiving.
Neural drive represents the effective amount of signaling to contract that a muscle receives from the central nervous system (CNS).
This also helps explain why a muscle can feel tight or stiff but still be weak. A sensation of tightness does not necessarily mean that the muscle has a high force-generating capacity. A muscle may maintain a relatively high level of neural activity and receive continuous signals to contract, making it feel tight or stiff, while its ability to produce force remains limited if it is operating at a non-optimal resting length.
