CONVENTIONAL IVF OR MINI IVF? THE ANSWER ISN’T ALWAYS WHAT YOU EXPECT.

A patient came to see me after four years of infertility and two unsuccessful IVF cycles at different fertility centers. The cause of infertility was bilateral tubal disease.

Previous IVF history

IVF Cycle #1 (February 2024):

* 15 antral follicles at baseline
* 6 eggs retrieved
* 6 Day-3 embryos, with 3 developing into Day-5 blastocysts
* All three blastocysts underwent Preimplantation Genetic Testing (PGT)
* 1 euploid embryo
* 1 mosaic embryo
* 1 aneuploid embryo
* Two embryo transfers, but no pregnancy

IVF Cycle #2 (2025):

* 16 antral follicles at baseline
* 8 eggs retrieved, but only 3 were mature (MII)
* Embryos cultured to Day 3
* Two embryo transfers, again with no pregnancy

Before discussing possible implantation factors, one thing was already clear: the ovarian stimulation outcomes were far from what we would expect.

She was still young, had an AMH level above 3 ng/mL, and a good antral follicle count (AFC). Naturally, she asked:

“Why did I retrieve so few mature eggs despite using stimulation medication? Should I switch to Mini IVF?”

After reviewing her previous records and evaluating her ovarian reserve, I concluded that she belonged to the good prognosis group—young age, high AMH, and adequate AFC. The stimulation doses used in her previous cycles were also appropriate.

So why had she shown a poor response in both IVF cycles?

To answer that question, I recommended FSH receptor (FSHR) polymorphism testing—a highly specialized genetic test that analyzes variations in the FSHR gene to predict how the ovaries respond to ovarian stimulation during IVF.

In clinical practice, this test can help us:

* Predict ovarian response to stimulation medication
* Personalize the stimulation protocol, including the most appropriate medication and dosage
* Explain why patients with the same age, AMH, and AFC may respond very differently to identical treatment protocols

As expected, the results showed that she carried an FSHR genetic variant associated with reduced ovarian responsiveness.

With this information, we confidently decided not to switch to Mini IVF. Instead, we proceeded with a personalized conventional IVF protocol, selecting the most appropriate medication and adjusting the dosage based on her genetic profile.

The outcome

This time:
✨ 16 antral follicles at baseline
✨ Excellent and synchronized follicular growth
✨ 15 mature eggs retrieved
✨ 8 high-quality Day-5/Day-6 blastocysts

The first challenge had been successfully overcome.

The next step is equally important: carefully reassessing all potential factors that may affect embryo implantation and addressing any underlying issues before embryo transfer.
