Carbon fibre UCBLs for Hypermobile Flat Feet

Swipe to see the difference.
Before Fitting: both heels rolling inwards, arches collapsed, and the classic “too many toes” sign from behind. This is pes planovalgus in an adult with joint hypermobility, where the ligaments simply don’t hold the foot’s alignment under load.

After Fitting: the same patient standing in his new orthotics. Heels vertical, arches supported, ankles stacked over the feet.

The devices are custom UCBLs (University of California Biomechanics Lab orthoses) laminated in carbon fibre. The deep heel cup and high medial and lateral walls grip the hindfoot and midfoot and hold them in a corrected position, rather than just propping up the arch the way a standard insole does.

Carbon fibre keeps the orthotics thin and rigid enough to control a hypermobile foot without taking up the whole shoe. Finished with a cushioned top cover for comfort.

For hypermobile patients, the goal is control: less strain on overworked tendons and ligaments, less fatigue, and a foot that doesn’t collapse a little more with every step.

A knee brace is only effective if it’s the right brace & fitted correctly.

Knee bracing can provide additional stability, support and protection, but there is no one-size-fits-all solution.

The appropriate brace depends on the individual, their condition, the level of support required and what they need to be able to do while wearing it.

Here, Malcolm takes a patient through the fitting process, ensuring the brace is correctly positioned, adjusted and comfortable.

A scoliosis brace shouldn't simply push a curve straight.

Scoliosis is a **three-dimensional condition**. The spine curves, the vertebrae rotate and the normal side-on profile of the spine can also change.

That's why the brace needs to work in three dimensions too.

At Malcolm Freedman & Associates, we fit the Rigo Chêneau type scoliosis brace, individually designed according to each patient's specific curve pattern.

The brace uses carefully positioned pressure areas together with expansion spaces that allow the trunk and ribcage to move towards a corrected position.

No two scoliosis curves are exactly alike - so their braces shouldn't be either.

Our aim is not simply to change a number on an X-ray. It's to manage the curve during growth while addressing overall trunk alignment and rotation.

Rigid carbon foot plates (full or ¾ length, ± Morton’s extension)
These work by limiting motion and redistributing load rather than storing energy. Main indications:

• Hallux rigidus/limitus — restricting 1st MTPJ dorsiflexion to reduce pain and progression
• Turf toe and sesamoiditis — splinting the 1st ray through propulsion
• Metatarsalgia, metatarsal stress fractures, and Morton’s neuroma (usually paired with a met dome)
• Midfoot and Lisfranc arthritis or post-injury instability
• Charcot midfoot — rigid offloading to limit rocker-bottom progression

Custom Supra Malleolar Orthoses (SMOs)

Fresh out of the workshop - a custom-fabricated pair of SMOs, made for one of our patients here at the practice.

An SMO is a lightweight orthosis that extends just above the ankle (the malleoli), giving control of the foot and ankle in the side-to-side plane while still allowing natural movement up and down. That balance is what makes it such a useful device.

We use SMOs to:
• Stabilise a foot that rolls inward or outward, bringing the heel and forefoot into better alignment
• Improve standing and walking balance in children and adults with low muscle tone, hypermobility, or mild neuromuscular conditions
• Support more efficient, confident gait without the bulk of a taller brace
• Provide a stable base for motor development in children learning to stand and walk

Because every foot is different, each device is moulded and built to the individual. A custom fit that off-the-shelf bracing simply can’t match. The goal is always the same: better function, better alignment, and a patient who can move more comfortably
