Breast Care Matters…

South Africa does not have a formal national breast cancer screening programme due to public health funding constraints, though breast cancer remains the most frequently diagnosed cancer in South African women.

Key Breast Cancer & Mammography Stats

- Breast cancer accounts for over 25% of all new cancer cases diagnosed in SA women.
- Roughly 1 in 25 to 1 in 29 SA women will develop breast cancer in their lifetime, with urban areas reporting higher incidence rates up to 1 in 8.
- The average age of breast cancer diagnosis for SA women under 40 is 34 years, and cases in women aged 40–49 have increased (rising from 19.4% to 26.8% in a major Johannesburg study).

Screening Guidelines

The Radiological Society of South Africa (RSSA) and the Breast Imaging Society of South Africa (BISSA) recommend annual mammograms starting at age 40.
Most private medical aid schemes cover mammograms only biennially (every two years) for average-risk members.

Did You Know?

Public-sector patients often experience later-stage presentation, larger tumour sizes at diagnosis, and longer diagnostic intervals compared to private-sector patients.

What Is A Mammogram?

It is a highly specialised, low-dose X-ray examination of the breast tissue. It allows a radiologist to capture clear, detailed images. 3D mammography improves visibility for women with dense breast tissue.

Do I Need A Mammogram?

Yes, it is the universal “gold standard” for early breast cancer detection. Extensive clinical evidence shows that mammograms provide significant life-saving benefits:

- A mammogram locates preclinical abnormalities and tell-tale changes in breast tissue years before physical symptoms or lumps develop.
- Regular screening mammography has been shown to reduce breast cancer deaths by 15% to 40% depending on age and adherence.
- Finding cancer early means smaller, localised tumours. This often translates to fewer mastectomies (breast removals), less extensive surgeries, and lower requirements for aggressive chemotherapy.
- Mammograms successfully identify roughly 87% of women who have breast cancer.

Phytohormones - an alternative to HRT?

Phytohormones (phytoestrogens like isoflavones from soy or red clover) are plant-derived compounds with weak estrogen-like structures.

How They Compare

- True HRT (like bioidentical estradiol and progesterone) directly replaces declining natural hormones. Phytoestrogens bind weakly to estrogen receptors or modulate them differently, and most dietary forms require specific gut bacteria (which only 20% of people have) to convert them into active forms like S-equol.
- Standard HRT is clinically proven to effectively treat severe menopausal symptoms like hot flashes and night sweats. Clinical evidence for phytoestrogen supplements relieving these climacteric complaints remains mixed.
- HRT provides established bone density and cardiovascular protection. Some studies suggest phytoestrogens may have mild anti-osteoporotic effects.
- According to SA consumer behaviour studies, pre- and post-menopausal women frequently prefer plant-derived therapies because they historically believed they carry fewer risks and fewer side effects than conventional pharmaceutical HRT.
- Local research indicates that 80% - 84% of urban SA women are aware of conventional medical HRT, a significant portion are reluctant to utilise it, opting instead for over-the-counter natural and phytoestrogenic alternatives. In contrast, up to 94% of women in rural areas rely entirely on traditional, plant-based remedies.

Safety & Regulation

- Herbal and plant-based alternatives are not tested, standardised, or regulated with strict safety and purity controls.
- The South African Menopause Society (SAMS) notes that while plant-based precursors are used to manufacture regulated bioidentical hormones, simply consuming unformulated plant products does not yield consistent therapeutic blood levels. SAMS guidelines explicitly caution that phytoestrogens (like genistein) / natural supplements (black cohosh), are not recommended for breast cancer patients.

Adaptability: A Powerful Tool Learning to accept change and inconsistency is one of the most powerful steps we can take ...

Adaptability: A Powerful Tool

Learning to accept change and inconsistency is one of the most powerful steps we can take for mental health. Culturally, we are often expected to be perfectly consistent, maintaining a steady mood, high productivity, and constant emotional availability.

Female biology and psychology are inherently cyclical and dynamic. True mental well-being comes from shifting the goal from perfection to adaptability.

1. Reframe Inconsistency as “Cyclical Nature”

View changing moods/ energy levels as natural rhythms.

- Throughout the menstrual cycle, pregnancy, postpartum, and perimenopause, shifting estrogen and progesterone levels directly impact brain chemistry and energy.
- Modern work and social structures are built on a 24-hour linear clock (traditionally matching male hormonal patterns). The female infradian rhythm operates on a roughly 28-day cycle.
- Practice acknowledging your current state without immediate judgment - “I am feeling low-energy today, and that is a valid state of being,” stops the exhausting cycle of feeling guilty for being human.

2. Move from Control to Adaptability

Anxiety thrives on trying to keep everything certain and constant.

- Rigid routines often break under the weight of life’s unpredictability. Aim for flexible routines that have built-in “buffer days” for when your mental health shifts.
- When external changes or internal moods feel overwhelming, separate what you can control from what you cannot. You cannot always control a sudden hormonal dip or a life transition, but you can control how gently you treat yourself through it.
- Embrace dialectical thinking. You can be anxious about a major life change andfully capable of handling it at the same time.

3. Practice Dynamic Self-Care

If your energy and mental state are inconsistent, your self-care cannot be one-size-fits-all. It must adapt to what your body and mind need in the present moment.

4. Build a Mindset of Self-Compassion

Accepting inconsistency requires unlearning the societal narrative that a woman’s worth is tied to constant productivity or being a perpetual caregiver.
