This scan takes seconds, touches nothing, and shows me the layers of your retina in detail once only possible under a microscope.

It is called an OCT, short for optical coherence tomography. You sit at the machine, look at a target, and it uses light to build a cross-section through the back of your eye. No dye, no needles, no radiation, nothing touches your eye.

Here is what makes it remarkable. Put an OCT scan next to a histological section, a real slice of retinal tissue viewed under a microscope, and the layers line up. We are getting something close to microscopic detail from a living eye, in a chair, in under a minute.

That changes what we can find. Fluid under the macula from diabetic eye disease. A macular hole. An epiretinal membrane pulling the retina out of shape. Early age-related macular degeneration. Swelling that has not yet affected your vision enough for you to notice.

It also changes how we follow you. When we treat, we can measure the response scan by scan rather than guessing from the vision chart alone.

Forty years ago this technology did not exist. Today it is a routine part of an eye examination.

Good news if you dread the air puff test: we do not use it.

Eye pressure is one of the most important numbers we measure, because raised pressure is the main risk factor for glaucoma, and glaucoma usually causes no symptoms until damage is already done. So the number needs to be right.

The air puff test, or non-contact tonometry, is a useful screening tool. It is quick, it needs no drops, and it flags patients who need a closer look. But it is a screening test, not a precision measurement, and in a specialist practice we need precision.

So we use two methods instead:

Goldmann applanation tonometry. The international gold standard. A numbing drop, a yellow dye, and a small probe that gently rests against the surface of the eye at the slit lamp. You feel almost nothing, and it gives us the most accurate reading available.

Rebound tonometry. A tiny, lightweight probe touches the eye for a fraction of a second. No drops, no air, very well tolerated. It is excellent for children, for anxious patients, and for anyone who cannot sit comfortably at the slit lamp.

Two methods, no air puff, and a number we can actually plan treatment around.

Suddenly reading without your glasses again? Don't celebrate yet. This "second sight" effect often isn't your vision improving, it's a classic early sign of cataracts changing the lens of your eye. If it happens suddenly later in life, it's worth getting checked.
