The goal of Primary Health Care (PHC) is to promote and protect health, providing accessible and equitable care throughout an indivual's lifespan, whilst empowering communities to take control of their own health and well-being. PHC provides comprehensive healthcare that goes beyond just treating specific diseases to include the promotion, prevention, treatment, rehabilitation, and palliative care of physical and mental health, so that people know what causes their illnesses, are enabled to prevent illness, and kept healthy as long as possible.

Erectile Dysfunction Management

What is it?
Erectile dysfunction (ED), also known as impotence, is the consistent inability/difficulty to achieve and maintain an er****on firm enough for satisfactory s*xual in*******se. While occasional difficulty with er****ons is normal and can happen to most men, persistent issues may indicate an underlying health concern and can have a significant impact on a man's self-esteem and relationships.

Symptoms of ED:
The primary symptoms include:
• Difficulty getting an er****on - even with s*xual stimulation (playing with it, massaging, foreplay, etc).
• Difficulty maintaining an er****on - Inability to keep an er****on long enough to initiate, complete or enjoy s*x
• Reduced s*xual desire - decrease in libido, where you do not feel like having s*x at all

Management - How We Treat Erectile Dysfunction:
Successful Management involves first understanding the root cause of the problem! This helps us to treat it sustainably, where the problem is eliminated permanently/long term.

Some causes may be:

Psychological:
- Everyday stress, as well as performance anxiety related to s*x
- Depression can affect libido and performance
- Relationship issues can crop up and affect s*xual engagement and relations

Physiological Causes:
These are very often the main cause and have to be investigated to be appropriately diagnosed.
• Cardiovascular disease - Conditions like heart disease, high blood pressure, high cholesterol, and atherosclerosis (hardening of the arteries) are the most common causes of ED. These conditions can restrict blood flow to the p***s.
• Diabetes - High blood sugar levels can damage nerves and blood vessels, affecting the efficiency of blood circulation, leading to ED.
• Neurological Conditions such as multiple sclerosis, Parkinson's disease, and spinal cord injuries can interfere with the nerve signals required for an er****on
• Hormonal imbalance - Low levels of testosterone or other hormonal issues can contribute to inability to ED
• Prostatism - an enlarged or inflammed prostate can affect blood flow to the p***s, and is a common cause of ED in men over 35yrs old
• Being overweight is a significant risk factor for cardiovascular disease and diabetes, and therefore also for ED
• Prescription drugs for conditions like high blood pressure, depression, and prostate issues can cause ED as a side effect
• Smoking, excessive alcohol consumption, and substance abuse can all negatively impact erectile function
• Pelvic injury or disease can affect the nerves and arteries, impacting blood flow to the p***s
• The development of scar tissue inside the p***s (from old, untreated STIs) can cause painful er****ons and contribute to ED

At Mi-Clinic, we will ask questions about your symptoms, lifestyle, and screen you for any medical conditions you may have.

We will also conduct urine and blood tests to detect underlying conditions such as diabetes, heart disease, or low testosterone.
We will conduct physical and psychological assessments to determine any contributing factors.

Treatment is based on the causes identified, and ranges from hormonal replacement therapy for those with inadequate levels, to lifestyle modification and oral medications that help to increase blood flow to the p***s.We also work closely with doctors and urologists that will remove any resultant scarring in the p***s from old injury or disease, or address prostate issues when they are present.
