Today's topic is on the comparison between a stationery oxygen concentrator and a portable oxygen concentrator(POC). In all cases the stationery concentrator must be your primary source of treatment. The portable concentrator must never be used whilst you sleep . The stationery concentrator produces a continuous flow rate whilst the POC produces a pulse flow rate. A POC offers you freedom because of its size and the batteries. The POC can also act as a backup during short power outages, in addition to your backup cylinder.
What most patients do not know is that not all of the oxygen from the stationery is absorbed by the patient. The concentrator delivers a continuous flow . Therefore some of the oxygen is wasted in the expiration cycle (when the patient breathes out) . The lungs do not absorb 100% of the gas that passes through them; healthy lungs typically transfer a fraction of the available oxygen into the bloodstream to raise blood oxygen saturation safely back to normal target ranges (usually 92%–98%).

The POC delivers in pulse doses, but the actual volume is lower . POC have settings of up to 6 but that does not mean 6LPM. Therefore, we discourage patients prescribed with high flow rates from buying a POC. The POC gives you freedom from your stationery concentrator. Studies have shown that mortality rates are lower for patients who regularly use their POC . However the experience we have gained is that the stationery flow rate must be below 4LPM for the patient to get the best out of the POC . It is without question that the prescribing Doctor must be okay with the patient using a POC. Some patients are even given the go ahead by their doctors to fly with their POC . We often get asked by our patients what is FiO2. FiO2 is the fraction of air that we inspire. In a room or outside the fraction of the oxygen is 21%. Therefore you can say Fraction of Inspired Oxygen at room air is 21%. On your blood results it sometimes shows as taken at RA or 21%.
When doctors prescribe oxygen, They would use the term for example, 28% FiO2. This means that the patient needs supplementary oxygen because the patient is unable to maintain saturation at room air. The example of 28% can also be stated as 2LPM, using a nasal cannula. Here are other flow rates based on nasal cannula.
• 1 LPM: 24% FiO2
• 2 LPM: 28% FiO2
• 3 LPM: 32% FiO2
• 4 LPM: 36% FiO2
• 5 LPM: 40% FiO2
• 6 LPM: 44% FiO2

There are times when the FiO2 delivered is not adequate. .This when under doctors instructions we can achieve a FiO2 of between 35% to 60% using a simple face mask. However this should be in a hospital environment. Whilst we can supply 10LPM concentrator, Those sort of flow rates indicate that the patient needs more monitoring.
We can also use a Non Breather and push the FiO2 up to 90%. In this case high flow rates of between 10 to 15LPM is required. Once again high flow rates like this should be in a hospital environment so that the patient can be monitored.
The condition that we treat with high flow in the home environment are Cluster Headaches. Here we use a cylinder with 99.99% purity medical oxygen . The doctor prescribe the flow rate to between 12 to 15LPM using a non rebreather mask. The recommendation is to start the oxygen therapy when feeling the headache starting and continue the therapy for about 25 minutes or until the pain goes away.

In severe cases, it can cause fluid to build up in the lungs and impair the body's ability to take in air.
