WHAT WOULD YOU DO FIRST?

A 28-year-old male was attacked by several individuals in the Sawmills area of Livingstone. During the assault, he was kicked repeatedly around the knee and fell heavily onto the affected leg.
Two days later, he was brought to the facility on a wheelchair because he was unable to walk.
On assessment:
🔹 The knee appears mildly swollen
🔹 There is significant tenderness on palpation around the knee
🔹 Both active and passive ROM are reduced because of pain
🔹 The patient is unable to weight-bear
🔹 No obvious open wound is noted
🔹 The patient reports that the pain has remained severe since the incident

📸 The pictures show the affected limb.

STOP UKUCHINA A FRESH INJURY! 🦵🏽

You twist your ankle, fall, pull a muscle or get hit… then the first thing you do is ukuchina with warm water, Lofnac gel or Strong Heat Cream?

⚠️ BE CAREFUL !

Immediately after a soft-tissue injury, there may already be bleeding and swelling inside the tissues. Heat increases blood flow, which can potentially make the swelling and inflammation worse.

❌ Avoid immediately applying:
-Very hot/warm water compresses
-Strong heat creams
-Heat rubs
-Hot massage

✅ FOR THE FIRST 24–48 HOURS:
-protect the injured area
-rest from aggravating activity
-use a cold pack wrapped in a cloth
-elevate the limb where appropriate.

🔥 Heat has its place—but timing matters!

That “ukuchina” your grandmother taught you may feel good, but feeling warm doesn't always mean the injury is healing. 😅

REAL-LIFE CLINICAL SCENARIO — WHAT WOULD YOU DO?

You are working at a rural health facility when a 39-year-old man walks into your department complaining of difficulty walking.
He tells you:
“I broke this leg a few years ago after an accident. It was treated, but since then my leg has never been the same.”

On assessment, you notice that the left thigh appears visibly deformed. The patient walks with an abnormal gait and reports pain after walking for a long distance. He also says he sometimes feels that the injured leg is shorter than the other leg.

You request an X-ray, and the images below are returned.
NOW PUT YOURSELF IN THE CLINIC
1. Based on the history, physical examination and X-ray, what do you think happened to this patient's femur?
2. The fracture appears to have healed, but the bone is obviously deformed. What complication of fracture healing are you suspecting?
3. Before prescribing any exercises, what 5 things would you assess in this patient?
4. How could this deformity affect his:
-Walking pattern?
-Hip and knee function?
-Muscle strength?
-Leg length?
-Balance?
5. As the physiotherapist at a rural facility, would you start rehabilitation immediately, or would you refer him to an orthopaedic surgeon first? Give your reason.

BONUS:
If this patient has been living with the deformity for several years, what long-term musculoskeletal problems could develop because of the altered biomechanics?

X-RAY CHALLENGE: WHAT DO YOU SEE?

A 28-year-old male presents to the clinic after twisting his right foot during a football match. He reports immediate lateral foot pain and difficulty putting weight on the foot.
On examination:
👉🏽 Significant tenderness along the lateral aspect of the foot
👉🏽 Swelling and bruising
👉🏽 Pain aggravated by weight-bearing
👉🏽 Reduced ability to walk normally
👉🏽 No obvious neurovascular deficit

An X-ray of the right foot is requested…
LOOK CAREFULLY AT THE IMAGE BELOW

QUESTIONS FOR THE CLINICAL TEAM:
1️⃣ What fracture do you see?
2️⃣ Which metatarsal is involved?
3️⃣ Is the fracture displaced or undisplaced?
4️⃣ What mechanism of injury could produce this pattern?
5️⃣ As a physiotherapist, when would you begin rehabilitation?
6️⃣ What complications should we watch out for?
⚠️ Don't just look at the X-ray—think clinically!
A fracture is not simply a broken bone. The location, displacement, stability, mechanism of injury and soft-tissue involvement all influence management and rehabilitation.
