Please be aware of changes to vitamin B12 testing in Australia!

There are some previous posts on this from early 2025 I believe, when Medicare changed the recommended vitamin B12 test to the (almost useless) Active B12 test to be the standard test. And they also changed, ie restricted/limited, the number of tests for B12 in a patient to generally only once in 11 months.

But, the Australian labs have also been told to change the reference ranges for the Active B12 test to ">25" (from ">35") as being "normal", thus making it even more difficult for anyone to ever be diagnosed with a vitamin B12 deficiency! Or to get a treatment for this very important and essential nutrient.

In the IIFP training courses, we recommend that the minimum optimal range for the Active B12 test is ">128", which is significantly higher than the ridiculous lab reference ranges. Hence why we can pick up a B12 deficiency much sooner, and be able to treat this deficiency for quicker improvements in our patients.

We also disagree that the Active B12 test should be the standard or recommended test anyway, especially on its own or without the much better "Vitamin B12" or "Total B12" test.

Why are there several Institute for Integrative and Functional Pathology (IIFP) pathology analysis training courses, and which one should you look at doing?!

At this time there are 3 levels of the IIFP pathology analysis training courses, which could be seen as:

1. Level 1 - the introduction to pathology analysis, including why and how you can use this in your clinical practice
2. Level 2 - advanced pathology analysis techniques, covering more tests, more biochemistry, and many clinical cases
3. Level 3 - expert/mastering of pathology analysis.

Why do we need to learn how to analyse and interpret blood test results as a Nutritionist, Naturopath or Herbalist?!

For a few main reasons:

1. Almost all new patients we see will bring blood test results to the initial consultation, and expect us to understand how to interpret them
2. Almost all patients will have already seen a GP to get these tests, and in almost all cases, the GP will say “everything is normal!”. This is frustrating for the patient as they get no answer for their issues or symptoms. It’s also NOT possible to have symptoms and health issues and having “normal” blood test results!
3. If a GP cannot find any issues I the test results to help a patient, then YOU need to be able to!
4. And because we get no formal training on pathology analysis in our qualifications.

Even if you are still a student (of Nutrition, Naturopathy, or even Herbal Medicine) you can learn a lot about how to interpret and analyse pathology results for your clients in your assignments, clinic subjects, or even your own pathology test results! As a student, you will be in a great position for analysing your clients' pathology results as soon as you start practicing after you graduate! You will be in a far better level of experience on pathology analysis than even myself on graduation, or other experienced practitioners, who were not trained in pathology analysis until years into clinical practice.

An iron deficiency occurs in stages, as the attached infographic shows. But you might not see these stages developing, as people don't often get regular blood tests, so we more likely see that they are well into anaemia and iron deficiency symptoms when they see us and we analyse their results.

However these stages and resulting changes in iron studies biomarkers are not always the same as in the infographic, as there are other factors which can affect the iron-related markers, especially of Ferritin. As a result, Ferritin isn't always the first biomarker to show the signs of a possible iron deficiency.

Hence the 4 stage progression, assuming no infection or inflammation or tissue damage, should be:

Stage 1 – the iron stores as Ferritin, starts showing as low

Stage 2 - the iron being available to be transported, being the Transferrin Saturation %, starts showing as low

Stage 3 – Haemoglobin, the oxygen transport protein in the red blood cells, starts showing as low due to the reduced iron

Stage 4 – MCV is usually the last of these biomarkers to show low iron, in our experience.

More often we would actually see that Transferrin Saturation starts showing as low first, and before low ferritin shows as low!

Also, a reminder to never just test for Ferritin on its own, or interpret iron levels from this one biomarker! In so many cases, we have seen low Ferritin in early to advanced stages of iron overload conditions (such as Haemochromatosis)! We have seen doctors and other practitioners recommending iron infusions and/or iron supplements in these cases of low Ferritin, when all other iron-related biomarkers are showing as very high iron overload situation.

Based on new studies, we should be using a new optimal minimum range for magnesium when interpreting blood test results!

The previous optimal minimum mentioned in the IIFP training and software was 0.8 mmol/L, but new info suggests that this is too low and should be increased to 0.85 mmol/L.

Please be aware of this in your pathology analysis, and for those who have and use the IIFP software, you should open your IIFP software template file and change the optimal minimum for magnesium from 0.8 to 0.85 and save this change! Then all your new patient files will use this new optimal minimum value.

This change will be added into the IIFP training course material.

High fasting serum insulin levels in a blood test can certainly suggest a high carbohydrate diet over a longer period of time. This can also suggest a possible “insulin resistance” issue if the serum insulin result is quite elevated, which can be a risk factor for the development of diabetes.

We know that blood glucose cannot get into a cell without the help of insulin to “open the door” to the cell. But even in the presence of high blood glucose and high insulin, glucose levels can remain high or get very high, which can lead to diabetes.

I’ve come across a different thought on this situation from a Professor of Human Physiology and Nutrition, Prof Bart Kay. He says that “insulin resistance” is not a pathogenic situation at all, but a protective mechanism!

We also know that having a high blood sugar level is very toxic in the body, as it causes high inflammation and oxidative damage to cells and organs and tissues. In situations of high blood sugar and insulin, the cells will already have enough unused/stored glucose and Prof Kay suggests that our cells know that high blood sugar causes oxidative damage and hence won’t allow any more glucose into the cell to prevent further oxidative damage. This is therefore a protective mechanism by the body and the cells!

Unfortunately this protective mechanism to stop oxidative damage then can lead to other symptoms and eventually to diabetes. I get his thoughts on this, but insulin resistance still occurs and will lead to further health issues. I also understand that insulin resistance is not a root cause of diabetes development, but it is a factor. But we still have to go back to the root causes of the high oxidative damage, high inflammation, high blood sugar and high insulin levels, and reduce or limit or even temporarily avoid carbs which is driving the whole chain of events.

This metabolic process of limiting fuel intake into a cell also occurs to prevent of fats for fuel if a cell is currently using carbs and has enough of this in the cell. This also happens if a cell has sufficient fats inside to use for energy - it won't take in any more fuel, including carbs, as the current priority fuel is fats. This is a metabolic process called the "Randle Cycle".

Does this different perspective of high blood sugar, insulin and oxidative damage make sense to you? What are your thoughts on this being a protective mechanism?
