We are incredibly proud of our Enhanced Nurse for Care Homes, Vicki. Not only does she consistently deliver outstanding care alongside our PCN care home team, but she has also recently represented the PCN at the South Tees Infection Prevention and Control Study Day.

Vicki presented the important work she and the team have been leading to improve the identification, review, and management of urine infections among care home residents. This is an important area of patient care, and her contribution highlights the dedication and experise within our team.

We’re looking for a compassionate Social Prescribing Link Worker to join our Greater Middlesbrough Social Prescribing Team, supporting people experiencing complex mental health needs.

Working across GP practices in the Greater Middlesbrough Primary Care Network, you’ll provide personalised, trauma-informed support and help people connect with services, activities and support in their community.
