Really valuable visit and discussion today with Dr Andy Knox around neighbourhood health, prevention and how community pharmacy, optometry, local organisations and the voluntary care sector can work more closely together to support patients earlier.
At NHL Pharmacy & Eyecare and Saha, this is something we are already actively building and implementing on the ground through community pharmacy, eye care, digital support, local outreach and trusted voluntary sector relationships.
A key part of this is connecting patients not only to clinical care, but also to the wider support that affects health and wellbeing, especially within areas affected by deprivation and health inequalities.
It was especially valuable to have Dr Andy Knox visit in person, meet the team, see the work live, and discuss how this type of joined-up neighbourhood care can be delivered practically within communities.
A lot of exciting conversations ahead around prevention, Pharmacy First, cardiovascular risk reduction, respiratory and hospital-to-home pathways, sleep pathways, eye care, community support pathways and using trusted local healthcare infrastructure more effectively.
