A quick reminder that the updated version of the Hypertension Case-Finding Service (HCFS) specification has come into effect today, ๐ง๐ต๐๐ฟ๐๐ฑ๐ฎ๐ ๐ฏ๐ฟ๐ฑ ๐ฆ๐ฒ๐ฝ๐๐ฒ๐บ๐ฏ๐ฒ๐ฟ.
It is imperative that you read the spec if you haven't already done so, and make sure your team is familiar with the changes too. Key changes include:
โฆฟ Patients are only eligible to access the service once every five years
โฆฟ Patients who have already been diagnosed with hypertension can no longer be referred into a community pharmacy for a blood pressure check
โฆฟ Four off-site clinic blood pressure clinics can be organised per year, per pharmacy but prior permission must be sought
