3. ويڪسين اسٽاڪ جي تصديق

مانيٽرنگ دوران اسٽاڪ رجسٽر ۾ درج ويڪسين جي مقدار ۽ ILR (آئس لائينڊ ريفريجريٽر) ۾ موجود ويڪسين جي مقدار جي ڀيٽ ڪرڻ تي ڪافي فرق نظر آيو. اها صورتحال رڪارڊ ۽ عملي اسٽاڪ جي درستگي بابت ڳڻتي جوڳي آهي.

سفارش: اسٽاڪ رجسٽر ۽ ILR ۾ موجود ويڪسين جي مقدار جي فوري طور تي ٻيهر تصديق ڪئي وڃي، فرق جا سبب معلوم ڪيا وڃن ۽ رڪارڊ کي حقيقت مطابق درست ڪيو وڃي.

4. ايڊوانس پروگرام

ويڪسينيٽر مظهر ٻرڙو جو ايڊوانس پروگرام تيار ٿيل هو، جڏهن ته ويڪسينيٽر نبي بخش رند جو ايڊوانس پروگرام تيار ٿيل نه هو.

5. مانيٽرنگ دوران سامهون آيل خاميون ۽ سفارشون

1. ويڪسينيٽر نبي بخش رند جي غير حاضري ۽ موبائل فون تي رابطو نه ٿيڻ بابت لاڳاپيل اختيارين کان وضاحت ورتي وڃي.
2. رفيوزل لسٽ ۽ زيرو ڊوز لسٽ فوري طور تي تيار ڪري اپڊيٽ رکيون وڃن.
3. ڪوارٽرلي مائڪرو پلان بنا دير تيار ڪري مڪمل ڪيو وڃي.
4. اسٽاڪ رجسٽر ۽ ILR ۾ موجود ويڪسين جي مقدار ۾ فرق جي فوري جاچ ڪري رڪارڊ جي درستگي يقيني بڻائي وڃي.
5. ويڪسينيٽر نبي بخش رند پنهنجو ايڊوانس پروگرام فوري طور تي تيار ڪري EPI جي مقرر ڪيل طريقيڪار موجب رڪارڊ مڪمل ڪري.
6. سمورا EPI رجسٽر، لسٽون ۽ لاڳاپيل رڪارڊ باقاعدگي سان اپڊيٽ رکيا وڃن ته جيئن روٽين اميونائيزيشن جي نگراني ۽ ڪارڪردگي بهتر بڻائي سگهجي.

6. نتيجو

آر ايڇ سي بلڙي شاهه ڪريم جي مانيٽرنگ دوران عملي جي حاضري، ضروري لسٽن جي عدم موجودگي، ڪوارٽرلي مائڪرو پلان جي کوٽ، ايڊوانس پروگرام جي اڻهوند ۽ ويڪسين اسٽاڪ جي رڪارڊ ۾ فرق جهڙيون خاميون سامهون آيون. لاڳاپيل عملي کي هدايت ڪئي وڃي ته ڄاڻايل خامين کي ترجيحي بنيادن تي دور ڪري رپورٽ ۽ رڪارڊ مڪمل ڪيو وڃي.

Measles Case Response and Mop-Up Activity Report

Location: Berag Colony and Zaur Colony, UC-1 TMK

Today, I visited the Measles Case Response and Mop-Up vaccination activity conducted in At Berag Colony and Zaur Colony, UC-1 TMK. The vaccination team, comprising Vaccinators Shafiqullah Shah and Abdullah Arain, along with LHW Shabana Soomro, conducted measles mop-up vaccination activities alongside routine immunization sessions.

During my visit, the team had already vaccinated 43 children. By the completion of the activity, a total of 103 children had received measles and routine vaccinations.

First, I checked the vaccines and observed that the cold chain was being maintained properly. Afterward, I reviewed all the relevant documents and records. A banner was also displayed at the vaccination session site, which was a very good practice. Overall, the team was performing its duties efficiently, and the activity was progressing very well.

During the activity in Zaur Colony, the team faced some challenges related to community mobilization. These issues were resolved after Senior Vaccinator Ahmed Zaur of UC-1 TMK was contacted and intervened. He demonstrated excellent coordination and strong relationships with the local community. Following his efforts, community members cooperated with the vaccination team and brought their children to the session site for vaccination.

The coordination between the LHW and vaccinators was also excellent. Their teamwork, community engagement, and commitment contributed significantly to the success of the activity.

Overall, today's vaccination activity was very successful, with proper cold chain management, documentation, community mobilization, and coordination among team members.

Well done, UC-1 TMK vaccination team! Keep up the excellent work and continue your efforts to protect every child through immunization.

مسٽر ڪاشف علي ميمڻ آئوٽ ريچ سيشن دوران مجموعي طور سٺي ڪارڪردگي جو مظاهرو پئي ڪيو. سندس ڪميونٽي سان بهترين هم آهنگي هئي ۽ ٻارن کي ويڪسين لڳائڻ کان اڳ والدين کي مؤثر انداز ۾ ڪائونسلنگ فراهم ڪري رهيو هو.

وئڪسين ڪيريئر ۾ HPV سميت سڀئي گهربل اينٽيجنز موجود هئا، گڏوگڏ چار ڪنڊيشنڊ آئس پيڪ پڻ موجود هئا. سيشن سان لاڳاپيل سمورا گهربل دستاويز موجود هئا. بهرحال، سيشن جو بينر لڳل نه هو. ويڪسينيٽر ان ڪوتاهيءَ تي معذرت ڪئي ۽ يقين ڏياريو ته مستقبل جي سيشنن دوران گهربل بينر لازمي طور لڳايو ويندو.

مجموعي جائزو: ويڪسينيٽر جي ڪارڪردگي اطمينان بخش رهي، تنهن هوندي به سيشن سائيٽ تي سمورن گهربل انتظامن کي يقيني بڻائڻ لاءِ بهتري جي ضرورت آهي.

14 آڪٽوبر 2026ع تي بي ايڇ يو وسي ملوڪ شاهه جي اي پي آءِ فڪس سينٽر جو مانيٽرنگ وزٽ ڪيو ويو.

وزٽ دوران ويڪسينيٽر مسٽر ڪاشف علي ميمڻ ايڊوانس ٽوئر پروگرام (ATP) مطابق فيلڊ ۾ وڃڻ لاءِ تياري ڪري رهيو هو، جڏهن ته ليڊي ويڪسينيٽر رڪارڊ اپڊيٽ ڪرڻ ۽ سينٽر تي ايندڙ ٻارن کي ويڪسينيشن جون سهولتون فراهم ڪرڻ ۾ مصروف هئي.

سٺا مشاهدا (Positive Observations):

* رجسٽر سٺي نموني سان سنڀاليل هئا ۽ انهن تي رنگين پيپر جي سٺي نموني ڪورنگ ٿيل هئي.

* ڪولڊ چين جي صورتحال تسلي بخش هئي.

* زيرو ڊوز لسٽون اپڊيٽ ٿيل هيون.

* گراف چارٽ ۽ ماهوار ڪارڪردگي رپورٽ پڻ اپڊيٽ ٿيل هئا.

نشاندهي ڪيل خاميون (Gaps Identified):

* ڪجهه رجسٽرن ۾ خاميون ڏٺيون ويون.

* ويسٽ مئنيجمينٽ رجسٽر اپڊيٽ ٿيل نه هو.

* ڪجهه اينٽيجنز جا اسٽاڪ رڪارڊ اپڊيٽ ٿيل نه هئا.

* پرمننٽ رجسٽر ۾ ڊيفالٽر ٻارن جي نشاندهي ٿيل هئي.

هدايتون: ويڪسينيٽر کي هدايت ڪئي وئي ته نشاندهي ڪيل سمورين خامين کي دور ڪري ۽ ايندڙ مانيٽرنگ وزٽ کان اڳ سڀني لاڳاپيل رڪارڊن کي مڪمل ۽ صحيح نموني اپڊيٽ ڪري.

Outreach Session Monitoring Report – Village Hero Kolhi, UC TMK-01 (Uncovered Area)

Today, I visited the outreach vaccination session in Village Hero Kolhi, UC TMK-01, an uncovered area. During the visit, the newly appointed vaccinators, Shafique Ullah Shah and Abdullah Arain, were performing vaccination activities under the supervision of Senior Vaccinator Ahmed Khan.

The vaccination session was organized at a suitable location, and the vaccination activities were being conducted very effectively. The community members were actively participating and bringing their children to the session for vaccination. There was excellent coordination and mutual understanding between the vaccinators and the local community.

During the monitoring visit, I reviewed the vaccination status, and the cold chain was found to be in good condition. I also checked the defaulter list and zero-dose list and conducted a cluster assessment in the village. Three children were identified as defaulters for the MR vaccine. These children were brought to the vaccination session and successfully vaccinated.

Despite the village being an uncovered area, the vaccinators' performance was found to be very good. The vaccination activities were progressing smoothly, and the overall performance of the team was highly satisfactory. The community's positive response and the vaccinators' dedication contributed significantly to the success of today's session.

Meeting and Refresher Training Report

Venue: DHQ Hospital, TMK Taluka: TMK Date: 09-10-2026

Today, an important meeting and refresher training session regarding the SEIR Application was conducted at DHQ Hospital, TMK, with the DFC.

The meeting was attended by DSV Sir Nasrullah Memon Sahib, TSV, and all vaccinators of Taluka TMK.

During the meeting, vaccinators were provided with refresher training on the use of the SEIR Application, data entry, daily work procedures, and other important aspects related to the application.

During the session, some vaccinators reported various technical issues and application-related problems. The DFC carefully checked these issues and resolved them. In addition, mobile phone and network connectivity problems were also examined, and the necessary guidance and solutions were provided.

The vaccinators were given practical demonstrations and guidance on how to address common issues encountered while using the SEIR Application. This training aimed to help them perform their daily tasks, data entry, and reporting more accurately and efficiently.

Objective of the Meeting:

The main objectives of the meeting were to enhance vaccinators' knowledge of the SEIR Application, resolve technical and network-related issues, improve data entry accuracy, and ensure better-quality and timely reporting.
