Her InBody: 52.9 lb of skeletal muscle, 83.3 lb of body fat, a basal metabolic rate of 1307 kcal. She is already sarcopenic. Her metabolism is already running slow. The "normal" lab result is often the most dangerous diagnostic endpoint, especially during the perimenopausal metabolic cliff.

He was already taking both of them. Still deficient. Still injured.

B12 and zinc. Both in his supplement protocol. Both low on labs. He'd been struggling to recover from the same injuries for months.

Intake and status are not the same question. Deficiency can persist despite supplementation when the gut isn't absorbing efficiently, stomach acid is low, or there's genetic variation in how those nutrients are metabolized. But the piece most people miss is that active tissue repair drives demand up sharply. B12 is required for DNA synthesis and cell turnover. Zinc is consumed directly in wound healing and immune activation. When the body is trying to rebuild, it burns through both faster than a standard dose replaces them. The standard medical model interprets thyroid function as a binary pass-fail test. If your TSH falls within range, you are told everything is fine. However, your biological systems do not operate in binaries; they operate in patterns that dictate metabolic capacity.

One lab result is a data point. Ninety days of data is a pattern. I work with patterns.

When I sit down to review a patient's case at Root Cause Functional Medicine, I'm not looking at cortisol. I'm looking at what cortisol is doing in relationship to HRV trend, fasting glucose curve, sleep architecture, and inflammatory markers across time. A single result in isolation tells me where you landed on one day. The pattern tells me what your system is actually doing and why.

That shift in how I read data changes everything downstream. A cortisol that looks normal at 8am doesn't stay normal in context. Against a 90-day HRV decline and a fasting glucose that's been creeping up, that same number reads completely differently. The result didn't change. The picture did.

Everyone is talking about personalized medicine right now. Most of it is a single lab panel with a customized supplement list. That's not personalization. That's a snapshot.
