Did you know ventilation plays a particularly important role during opioid-associated cardiac arrest?

Opioids can suppress respiratory drive, leading to severe oxygen deprivation before cardiac arrest occurs. Research presented during a SafeBVM workshop discussed many important clinical trials. One clinical trial showed that CPR combining chest compressions with ventilation was associated with 2.44X higher odds of survival than compression-only CPR.

The takeaway: When opioid toxicity is involved, restoring circulation is not be enough. Delivering effective ventilation is critical.

Consistency When Every Breath Matters | Hoover Fire Department

Manual ventilation can be difficult to standardize during a high-stress cardiac arrest. Adrenaline, provider handoffs and differences in training can lead to breaths that are delivered too quickly or forcefully—and monitors alone still require someone to recognize and correct the problem.

This case study explores how Hoover Fire Department incorporated Sotair® into its existing BVM workflow. During routine weekly training, providers quickly learned to respond to the device’s tactile and auditory feedback by delivering slower, gentler breaths while maintaining appropriate chest rise.

The department reported:

• Less need for supervisory coaching
• Greater confidence in providers across experience levels

During this FDIC discussion, panelists break down three essentials:

Flow-Limiting vs. Pop-Off Valves | Why the Difference Matters

These valves do not function the same way—and the distinction becomes especially important when ventilating noncompliant lungs.

In this clip from his MHSRS 2026 presentation, “Feasibility of Prolonged Manual Ventilation,” CAPT Mark F. Brady explains how a flow-limiting valve responds when a breath is delivered with excessive flow. Instead of releasing gas like a pop-off valve, it temporarily limits the flow, acting as a forcing function to help providers ventilate within targeted pressure and tidal-volume parameters.

For patients with reduced lung compliance, including those with ARDS, higher pressures may be required to deliver the intended tidal volume. Understanding how each valve functions can help providers make more informed decisions during manual ventilation.

Did you know adequate ventilation during out-of-hospital cardiac arrest was associated with up to 4X greater survival?

But another study raises an equally important point: smaller doesn’t automatically mean safer.

Among 1,994 adult OHCA cases, return of spontaneous circulation occurred in:

• 40% of patients ventilated with standard adult BVMs
• 33% of patients ventilated with small adult BVMs

The takeaway? Avoiding overventilation matters—but so does avoiding underventilation. Bag size alone cannot ensure that every breath is adequate.
