A quick look at the bladder ultrasound protocol.

The bladder is systematically swept in sagittal and transverse planes to evaluate its overall appearance and check for internal debris, possible stones, wall irregularities or other abnormalities.

The protocol also includes evaluating the ureteral jets, measuring the bladder wall and calculating bladder volume. When indicated, pre- and post-void measurements are obtained to evaluate for residual urine.

It’s not only about measuring the bladder, it’s about carefully evaluating everything within and around it.

How do you differentiate the ECA from the ICA? 👀

Start at the carotid bifurcation in transverse and identify both vessels. Then look for these clues:

• The ECA has branches in the neck
• The ICA normally has no branches in the neck
• The ECA typically has a more pulsatile, high-resistance waveform
• The ICA has a low-resistance waveform with more continuous forward flow during diastole
• A temporal tap can produce small rhythmic deflections in the ECA spectral waveform

Remember: vessel position can vary, so don’t rely only on whether it appears anterior or posterior. Use the anatomy, branches, color flow and spectral waveform together. The temporal tap can be helpful, but it shouldn’t be used as the only identifying feature. Research on the temporal-tap maneuver⁠

In this video, I’m scanning myself while demonstrating part of a basic carotid ultrasound protocol. I focused mainly on B-mode imaging, with limited color Doppler to visualize the ECA and its branches.

Basic protocol:
Carotid Ultrasound Protocol

Transverse:

• CCA: proximal, mid and distal
• Carotid bulb and bifurcation
• B-mode and color Doppler images

Longitudinal/Sagittal:

• CCA: proximal, mid and distal
• Carotid bulb
• Proximal ECA
• ICA: proximal, mid and distal
• B-mode, color Doppler and pulsed-wave (PW) spectral Doppler at each level
This video does not demonstrate every image or component of a complete carotid duplex examination. Protocols may vary by facility.

Have you ever felt a lump in your neck and wondered what it could be? 👀

In this video, I’m showing you the lymph node that I can feel in my own neck and how it looks on ultrasound. I also used color Doppler to evaluate the blood flow in the area.

A soft-tissue ultrasound can help evaluate a palpable lump beneath the skin and provide useful information about its appearance, size, and location. Not every lump is something serious, but any new, persistent, growing, or painful lump should be evaluated by a healthcare provider.

Ever wondered what simple breast cysts look like on ultrasound?

In this educational example, you can see multiple simple cysts as I scan through the breast tissue. Simple cysts are fluid-filled sacs that typically appear completely black (anechoic) with smooth, thin walls on ultrasound.

Ultrasound is a valuable tool that helps evaluate breast tissue and further assess areas of concern. Not every breast lump is a cyst, and not every cyst requires treatment, that’s why proper imaging and interpretation are so important.

Pancreas visualization tip!

One simple trick that can make a big difference: ask your patient to push their belly out (belly breathing) instead of sucking it in. This can help shift bowel gas and improve your acoustic window, making the pancreas much easier to visualize.

Small adjustments like this can save time and improve your exam! 👏
