OPEN POSITIONS

✔ Intake Coordinator & Chronic Care Management (CCM) Coordinator.

RESPONSIBILITIES
Answer incoming phone calls with professionalism and care
Coordinate patient care and services
Schedule Telehealth, Office, House Calls appointments for Nurse Practitioners
Support care delivery across:
🏡 House Calls
🏥 In-Office Visits
💻 Telehealth
📞 Phone Encounters
QUALIFICATIONS
Fluent in English (required)
Additional languages highly preferred:
Armenian
Russian
Spanish (especially valued due to our rural patient population)
Strong organizational and communication skills
Passion for helping underserved and vulnerable communities
