Behind every number is a patient, a family, and a community we had the privilege of serving.

In 2025, Central Wyoming Hospice & Transitions cared for 538 patients across 17 Wyoming counties, providing 23,125 days of hospice care. On any given day, our team cared for an average of 62 patients, the equivalent of 2.5 rural hospitals.

That kind of care is only possible because of the people behind it. Our staff, volunteers, supporters, and community continue to show up for families when it matters most.

To everyone who helped make 2025 a year of compassion and care, thank you. We are grateful to be With You. 💜

Board of Directors Spotlight 💜

Meet Sarah Reed, one of the dedicated board members who helps guide the mission of Central Wyoming Hospice & Transitions.

By day, Sarah serves as an Officer and Retail Manager III with First Interstate Bank, a role she has proudly held for 28 years. Her career has been built on strong leadership, operational excellence, and a genuine commitment to the people she serves. She's known for developing high-performing teams, creating a culture of collaboration, and leading with integrity and a people-first mindset.

That same heart for service is exactly why Sarah brings so much to our Board.

Outside of work, Sarah gives back to her community and stays active in her church. She treasures time with family and friends and loves traveling whenever she gets the chance.

One of the biggest misconceptions about hospice is that it’s only for the very end.

The truth is that hospice can provide months of support, comfort, education, and resources for both patients and families.

The earlier families receive support, the more opportunity they have to focus on what matters most: time together.

We're excited to share a milestone for TCN and our member organizations. At our recent Growth Immersion, September 22–24, attendees had their first chance to earn the new TCN-Certified Growth Professional designation.

Certified growth professionals have shown a consultative sales approach built on integrity, service, and real partnership with the medical community. They can also clearly explain the value of appropriate hospice use to referral partners, eligible patients, and families.

The designation builds credibility with referral partners and across the TCN network.

If you've ever walked into one of her luncheons, you already know: the room is packed, the energy is warm, and that's no accident. It's Dana.

She manages 100+ volunteers with a kind of care that makes every single one feel seen and valued, not just scheduled. Ask anyone who's worked alongside her and you'll hear the same thing: she's the best coordinator they've ever had.
