Community Health Alignment Initiative (CHAI): In 1 year an estimated $7,318,886 was saved by Community Health Workers addressing health and social needs in your communities.

44 CHWs across the state provided 7,470 supportive services to 5,761 uninsured and underinsured individuals. CHAI’s CHW workforce generates a substantial financial value for the Palmetto State while helping families with their unmet health and social needs. Additional funds will allow expansion to additional communities and families, incurring even greater savings.

Andrea Lee, Associate Director of Operations and CHAI Regional Site Coordinator with the Center for Community Health Alignment, attended the Access Kershaw 15th Anniversary Celebration in Camden, SC. As part of the Community Medical Clinic of Kershaw County, Access Kershaw continues to play a vital role in improving access to care through care coordination, navigation services, and support for uninsured and underserved residents across the region.

We are proud to partner with Access Kershaw through the Community Health Alignment Initiative, which has supported the expansion of Community Health Worker (CHW) services into Lancaster County. This partnership continues to strengthen community-based support and improve connections to healthcare and essential resources for residents who need it most.

The Community Health Alignment Initiative (CHAI) Advisory Committee recently met to review Center for Applied Research and Evaluation (CARE) evaluation findings, discuss updates to CHAI data collection processes, and reflect on participant interview findings. Committee members provided feedback on strategies to increase awareness of CHAI’s impact, strengthen communication with stakeholders across South Carolina.
