You can eat perfectly, work out five days a week, and still be metabolically unwell if your cortisol is running the show.
Here's what most people don't realize: stress isn't just a feeling, it's a hormone cascade. When cortisol stays elevated, chronically, day after day, it tells your body to hold onto visceral fat, break down muscle for fuel, spike blood sugar, and disrupt the very hormones — estrogen, progesterone, testosterone — that keep everything else in balance. It's like leaving your car's engine revving in park all day. Nothing is moving forward, but you're burning through fuel and wearing down parts you didn't even know were connected.
Here's why this gets missed so often in conventional medicine: standard cortisol testing is built to catch disease at the extremes, conditions like Addison's or Cushing's, where cortisol is severely too low or too high. A single morning blood draw is genuinely useful for that. But it was never designed to detect the in-between pattern most people actually live in: cortisol that's chronically elevated or dysregulated from everyday stress, without ever crossing into a diagnosable disease. If the lab isn't flagged, the system says nothing is wrong, even though the physiology tells a different story.
This is why I never treat stress as a "soft" wellness topic. In my practice, unmanaged stress shows up as unexplained weight gain around the middle, stalled labs despite a clean diet, worsening perimenopausal symptoms, and sleep that doesn't restore you no matter how many hours you get.
You don't need to eliminate stress, that's not realistic. What matters is giving your nervous system a daily off-ramp: a consistent wind-down routine before bed, protein-forward meals that prevent blood sugar swings (which amplify cortisol's effects), and even five minutes of intentional breathing before you check your phone in the morning.
