📝 Registration & Check‑In Reminder

Intake registration is required for every office and virtual visit and is completed through Phreesia, our secure platform. This process allows us to confirm demographic and insurance information needed for your appointment and for accurate billing, including copays or deductible amounts determined by your insurance plan. Registration is sent three (3) days prior to your visit for convenient completion, supporting timely care, claims processing, and billing. Additional health screeners may also be requested to support safe and appropriate care.

⚠️ Policy Reminder

We kindly ask families to remain familiar with our Missed Appointment Policy and Vaccination Policy. Missed or late‑cancelled appointments may result in a fee. Repeated missed appointments or ongoing noncompliance with practice policies may also impact a patient’s ability to remain established with the practice. These guidelines help us provide timely and consistent care for all patients.
