📅 Booking Your Appointment
• Appointments are scheduled online through my booking link on my website https://www.evolvetattooremoval.biz
• A $50 consultation deposit is required to reserve your appointment.
• Your deposit will be applied toward your first treatment if you proceed.
• If treatment isn’t recommended, your deposit will be fully refunded.
• Deposits are forfeited for cancellations within 24 hours of appointment, no-shows, or failure to meet the appointment requirements outlined below.

Please read all confirmation emails and text messages carefully, as they contain important information regarding your appointment.

📝 Required Paperwork
Electronic intake forms, medical history, and consent documents must be completed at least 48 hours before your appointment.
This allows me to review your medical history, prepare for your visit, and spend your appointment focusing on you—not paperwork.
Failure to complete your paperwork by the deadline may result in cancellation of your appointment. Rescheduling is at my discretion. If cancellation occurs within the 24-hour cancellation window due to incomplete paperwork, your $50 deposit will be forfeited.

A valid credit or debit card is required to book and remain securely on file.
Declined or invalid payment methods may result in appointment cancellation and applicable administrative fees.

Your appointment time is reserved exclusively for you. Late cancellations and no-shows affect both my schedule and other clients waiting for appointments, so scheduling policies are consistently enforced.
