✨ Follow individualized care plans and agency policies

Indiana: The Aged & Disabled (A&D) Waiver pathway includes Health & Wellness for eligible individuals under 60 and PathWays for Aging for those age 60 and older. Structured Family Caregiving support may also be available.

Arizona: ALTCS may support eligible individuals who need nursing-facility level of care, with services delivered at home or in the community. Ask your care manager about assessments and available in-home hours.
