In this systematic review and meta-analysis stratified by indication and study design, semaglutide use was associated with a modest increase in NAION incidence in observational studies of patients with diabetes (26.7 vs. 18.9 per 100,000 person-years) but not in RCTs or in patients using semaglutide for weight loss, suggesting the NAION risk is low in absolute terms and, if present, limited to patients with diabetes.
