Today we’re celebrating the partnership between Amanda Trainer, Nurse Practitioner with MyHome Healthcare, and Mary, one of the dedicated nurses at Charter Senior Living of Danvers.

When our providers and assisted living nursing teams collaborate, residents benefit from timely communication, coordinated care, and a proactive approach to their health. By working together, we’re able to identify changes early, address concerns promptly, and help residents continue living safely and comfortably in the place they call home.

Changes in an older adult’s health often happen gradually, and sometimes the signs are easy to miss.

A few more falls. Difficulty managing medications. Increased fatigue. Changes in memory, appetite, or daily routines. These may seem like small changes, but they can be important signals that additional support may be needed.

At MyHome Healthcare, we believe proactive care starts with understanding the whole person, not just a diagnosis. Because we provide primary care in the home and within senior living communities, our team can help identify changes early, provide continuity of care, coordinate with care teams and families, and support residents in maintaining their health and independence.
