Hard work pays off! 🎉 Join us in celebrating a phenomenal health milestone.

One of our program participants started her journey with an A1c of 10.6%. Through dedication to health education, continuous support, and meaningful lifestyle changes, she successfully brought her A1c down to 7.9%!

Thanks to HCPC Patient Fund! ❤️ we were able to provide her with a glucose monitor and an air fryer, giving her the exact tools she needs to keep cooking healthy meals and managing her diabetes with confidence.

Progression, not perfection. We are so inspired by her journey and grateful for the resources that make these victories possible!
