Low-Dose Naltrexone (LDN) is getting attention as a possible immunomodulatory option in autoimmune and inflammatory conditions.

At these lower doses, it works differently — helping modulate the immune system and reduce inflammatory signaling. Research suggests it may increase endogenous endorphins and may also affect inflammatory pathways.

The strongest clinical data is in Crohn’s disease: a 2018 Cochrane review found current evidence still limited and based on small studies, but one small trial suggested LDN may improve both clinical response and endoscopic response in adults with active Crohn’s.

For rheumatoid and seropositive arthritis, observational data and reviews suggest LDN may be associated with reduced use of analgesics and other medications in some patients, though the evidence is still considered preliminary.

In terms of safety, LDN is generally described as well tolerated. Reported side effects include sleep disturbance, vivid dreams, headache, nausea, and fatigue.
