Modified cranial closing wedge osteotomy for cranial cruciate ligament disease with extreme tibial plateau angle with procurvatum. The pre op TPA was 63 degrees and we did get some tibial shortening even with the modification of sliding the apex of the wedge cranial to the caudal cortex, but we regained functional limb length by getting the tibia back under the femur.

Tibial Plateau Leveling Osteotomy is our most often used technique for stabilizing the knee of patients with cranial cruciate ligament disease. TPLO produces consistently good and durable results in most patients with the condition, including almost all small patients. Occasionally, a patient’s anatomy calls for a different approach. This patient has cruciate disease with an excessive tibial plateau angle and proximal tibial procurvatum. Rotating the proximal tibia is not possible to the extent that would be necessary and doesn’t address the curvature of the proximal tibia. Modified Cranial Closing Wedge Osteotomy was elected to acheive tibial plateau leveling while improving tibial conformation and stifle biomechanics.

Look out! This one is long and nerdy! Having a good success rate in surgery is about ex*****on, but also about making good choices before the procedure. One such choice is “patient selection”, making sure the patient is well suited to the treatment and in a condition that minimizes the chance of complications. In this video, we discuss patient selection for TPLO, specifically in regards to size, weight, body condition and body type.

Check me out on ‘s Veterinary Leadership Success Show! I was talking with the good doctor about how to chart a course to expertise and how to avoid some hazards along the path that can conspire to make client interactions, and work in general, all too burdensome. The title might be jarring to all of my clients that follow me, but the whole moral of my story (if you don’t end up hearing the podcast) is that the idea of a challenging client as a defense mechanism that turns the opportunity to hear/help/educate someone into a lose-lose proposition. I think this is a good one for vet students (please share!), but I hope all who listen enjoy! Link below…
