MVP Health Care and Westchester Medical Center have announced a new partnership designed to strengthen care coordination during a hospital stay and beyond. By bringing MVP clinical care managers alongside hospital care teams, support continues as members transition home and move into recovery.

The partnership helps close gaps between hospital and home while expanding access to advanced and specialty care for MVP members across the Hudson Valley—supporting stronger outcomes and a more connected care experience.
