Therapeutic Insight: The Myofascial Release Perspective

Much of what we were taught is obsolete, since it was based
on outdated and inaccurate scientific assumptions. We were
taught the fascial system was just packing material and the
covering of muscles, but this information was based on
research on cadavers. The only way we have been able to
separate muscle from fascia has been done in our
imagination; it cannot be done in vivo.

Cadavers are brittle, so the focus was on the fibrous
network. Other forms of myofascial release mechanically
attempt to break up the cross links that can form in the
fibrous network. Unfortunately, this only partially releases
myofascial restrictions, so there are only temporary results.

What is missing? Our body is more than 70 percent fluid, and
the fascial system is the container and transport medium of
the body& #39;s fluid. Ideally, the ground substance of the fascial
system is a fluid to gelatinous state. The ground substance
is the very environment of every one of the trillions of cells in
our body, and recent research has shown there is a micro-
fascial system within each cell of our body.

My approach to myofascial release treats the biomechanical
aspect but also uses different fluid dynamic principles that
address our fluidity, which tends to solidify from trauma or
thwarted inflammation processes.

Yet, there is more. What has also been ignored by science?
Consciousness! How could that be? Please refer back to

Many scientific discoveries have suggested that
consciousness may be different frequencies of light and
sound that flow through the fluidity of the ground substance
and the microtubules of the fascial system. Light and sound
flowing through the body& #39;s fascial system is a relatively
foreign concept in traditional science and medicine.
Physiology, biology and neurology textbooks need to be
revised.

Perception
It has been known for a long time that if you and I and all of
the billions of people on Earth were to look at a mountain
range, each of us would perceive it differently. The colors,
shapes and textures, as well as the emotional and mental
impact, are not out there, they are within us.

What we perceive through our senses are vibrations of
energy. The neuromatrix theory describes this concept well;
however, proponents of this theory have been myopic and
seem to think that the brain and nervous system hang in
suspended isolation. This reductionist view is incomplete
and inaccurate in that it tells us to just consider ourselves
thinking brains whose role is to figure everything out
through logical thought.

Contrary to the reductionist view, the myofascial release
perspective teaches that we are feeling minds. Our mind is
not just in our brain as we were taught. Our mind is in every
cell of our body and beyond, a part which is our
subconscious. Neuroscientists have discovered the
databases in our subconscious are in excess of 10 million to
one of that of the consciousness of our brain. So instead of
us just being thinking brains, we are also feeling minds
whose function is to perceive the sensory information that
bombards us moment by moment. The vibrations of this
sensory information are sent through the fluid within the
billions of microtubules of the fascial system at enormous
speed. These vibrations stimulate the neural system and
brain that reduce this vast amount of sensory information
and convert these frequencies into symbols we call words,
thoughts, colors, textures, emotions and meaning.

To summarize, vast amounts of vibratory information and
wisdom flow through the fluidity of the fascia& #39;s mind-body,
within which lies the neuromatrix system. These incredible

systems function together as a whole mind-body response
and cannot be separated in vivo.

While this is interesting to write about, the profound effects
of myofascial release cannot be truly understood until one
experiences it. Only through your direct experiences with
myofascial release will you understand how it can help you
lead a pain-free, active lifestyle, with inner tranquility and
mental clarity. What I have tried to do in my close to 50-year
career treating people from all throughout the world is create
an intelligent model of reality and treatment that is gentle,
safe and effective.

The myofascial release perspective offers us and our clients
hope and a return to ease in our lives. This is an exciting
time to be a therapist.

Therapeutic Insight: The Myofascial Release Perspective
The Web of Life

I consider the liquid crystalline lattice of the fascia to be the web of life. Without the fascial web and the fascia's extra cellular matrix (ECM), life would not be possible.

Bob Mollica, P.T., references the following article, An architect walks into the lab. The following quotes are comments from Marilyn on my blog titled, Structure vs. Function: "I liked how the article addressed the importance of the ground substance and acknowledge it has been overlooked by teachers. I love the following explanation and wish I had been taught this concept early on in massage school.

"The average high-school biology teacher probably doesn't spend much time on the extracellular matrix, but animal life would be all but impossible without it. The extra cellular matrix (ECM) is the connective tissue that provides structural support to living cells, giving them a sort of scaffolding to which they can anchor. It also regulates communication between cells, stores and releases chemicals that can trigger a range of cellular functions, and governs the movement and migration of cells through its intricate architecture. The complexity of the system beggars description. The components of extracellular matrices are manufactured inside of their resident cells, which then fall subject to the influence of structural and biochemical changes within the scaffolds they have excreted. The far-reaching influence of the ECM has profound implications. 'You could potentially treat diseases and cause cell behavior to change by changing the extracellular matrix's elasticity,' this is the benefit of myofascial release therapy. Amazing!
I combine many massage and energy techniques with acupuncture as I use myofascial release, which is a very powerful combination. I initially used a neuroprobe, electronic acupuncture and cold laser on acupuncture points. I eventually flew to Hong Kong to learn as much as I could.

In Hong Kong, I learned how to use my fingers on acupuncture points to stimulate the flow of qi (energy) along the acupuncture meridians. I eventually began to apply myofascial release principles to the acupuncture system with great results.

I believe the acupuncture system meridians lie within and are profoundly influenced by the fascia's ECM. The following quotes are from my first book, Myofascial Release: The Search for Excellence. Another important component of our mind-body connection is the transmission of our body's bioelectrical currents along the three-dimensional network of the fascial system.

Increasingly, medical researchers and experienced health professionals are beginning to view the body as a self-correcting mechanism with bioelectric healing systems. According to one author, while some scientists are starting to explore the body's sensitivity to electromagnetic energy, those of the old school still choose to ignore the matter because it upsets their long-held theories. Such an attitude is unwarranted, however, and may also be unhealthy. It is known that electromagnetic fields 'trigger the release of stress hormones, [and} can affect such processes as bone growth, communication among brain cells and even the activity of white cells

"Two authors found naturally occurring direct-current signals that they called 'the current of injury' (COI). These signals are thought to be transmitted by the sheaths of Schwann and glial cells that surround their neurons. Others, however, consider the body's healing currents to use the microcapillary systems. In this theory, the bioelectric circuits are turned on when membrane conductivity closes down, and the electric flow then takes the path of least resistance through the bloodstream. The mechanism had not been determined, but clearly, the body sends electricity wherever its healing effects are required.

"It is not clear if the body interprets the energy of many of our modalities and manual techniques as intrusive and therefore to be resisted. If such is the case, the body recognizes this resistance as important to its survival. Many therapeutic efforts to heal have, in fact, caused patients discomfort. This may begin to show why microcurrents and the gentle, sustained pressures of myofascial release produce results when conventional modalities and massage techniques have failed. If the body does not view these as intrusive, it is not compelled to resist. Instead, the techniques are accepted as assistive, allowing the organism's self­corrective mechanisms to be facilitated.

"This is not to say that conventional modalities, exercises, massage and other hands-on techniques are not valuable; certainly they are. It means that to treat the body comprehensively, the approaches must be combined, as they go together in a complementary fashion and produce consistent results.

"Copper wire is a well-known conductor of electricity. If copper wire becomes twisted or crushed it loses its ability to conduct energy properly. It is thought that fascia may act like copper wire when it becomes restricted through trauma, inflammatory processes, or poor posture over time. Then its ability to conduct the body's bioelectricity seems to be diminished, setting up structural compensations and ultimately, symptoms of pain or restrictions of motion.

"The diminution of our fascia/ systems ability to conduct energy may be due to melanin. Melanin is present in copious quantities in the fascia, and neuromelanin is present in the neural structures and brain, which are encased by fascia all the way down to the cellular level. Melanin has superior conducting properties at room temperature and is synthesized in mast cells, also found in the fascia, which influence the immune system. As a superconductor, melanin may regulate firing of nerve cells. It seems centrally involved in control of all physiologic and psychologic activity. The neuromelanin-neuroglial system is the major site of mental organization. The nervous system is made up principally of glial cells. These cells have electrical properties that appear to be a responsible for the piezoelectric phenomenon.

"Piezoelectric behavior is an inherent property of bone and other mineralized and nonmineralized connective tissues. Compressional stress has been suggested to create minute quantities of electrical current flow.

Like untwisting a copper wire, the myofascial release techniques can enhance the function of the acupuncture meridians and the fascia's ability to conduct bioelectricity, thus creating the environment for enhanced healing. They also can structurally eliminate the enormous pressures that fascia/ restrictions exert on nerves, blood vessels, and muscles and cells.

Myofascial release can restore the fascia's integrity and proper alignment and, similar to the copper wire effect, can enhance the transmission of our important healing bioelectrical currents within the acupuncture system."

Recent research has shown that every acupuncture point is a fascial structure, and when the needle is pulled out, called the snap, it stimulates the piezoelectric phenomena that sends qi, or energy, through the acupuncture meridians.

Piezoelectricity is a Greek word for pressure electricity, which is stimulated via myofascial release.

I view acupuncture meridians as the main rivers that carry the important qi throughout the body. In addition, I feel we have to consider that the billions upon billions of microtubules of the fascial system are the important tributaries of fluid and energy that feed into the main rivers (meridians).

Within each microtubule of the fascial system is fluid and energy, or qi, that flows through the liquid lattice, energizing the entirety of the structure. Myofascial release adds a significant extra dimension of effectiveness to acupuncture, massage, bodywork and energy techniques.

Years ago I was the featured speaker at a large TMJ symposium. There were hundreds of TMJ specialists, physicians and therapists in attendance. I always insist on following my theoretical lecture with a hands-on workshop; one must experience myofascial release to understand it. Without one's direct feel and experience of myofascial release creates an inaccuracy and empty theorizing which only leads down a blind alley.

When I entered the room where the workshop was held, to my surprise there were no tables. For a moment I wasn't sure what to do. All we had were chairs. Having no option, I created a technique on the spot.

The acupressure point LI 4 (long intestine 4, which is also called the Hoku point) is located in the web between the index finger and the thumb. Try this: Slowly push your opposite thumb into the web, and you will find a tender, hard spot.

Since this was a seminar for dentists whose focus was TMJ pain and headaches, I instructed them to have their partner sit on a chair, push their thumb into the Hoku point (acupuncture) and then add in the myofascial component by slowly pulling the arm until it is gently taut. You then have the client side bend and rotate their head away from the arm that is being pulled.

The time factor for a true myofascial release is very important. Hold the gently taut arm and the Hoku point for approximately three to five minutes. Try this on some of your clients with arm, shoulder, cervical TMJ pain and/or headaches. Let me know what kind of a response you have in the comment section.

The funny part of the story is that years later in another symposium, a dentist approached me and introduced himself. He explained he had been in the previous symposium and thanked me. He said he went back to his practice and tried the techniques I had taught him on the chair with incredible results. Now, he and his assistants use it every day and his patients love it.

I had totally forgotten about that experience. A good lesson: When adversity presents itself, be creative.

Myofascial Release and TMJ Therapy

In the 1970s and 1980s, TMJ syndrome established itself in the
medical field as a serious disorder. Patients presented with moderate to severe pain symptoms with tension headache, migraine, ear pain, jaw pain, neck pain, tinnitus, dizziness and a host of other symptoms. Invariably they had been examined by many doctors at the best medical centers without avail. The best technology could not decipher their problem.

The TMJ field has come a long way since Costen reported his finding in the 1930s. Work has been presented showing the importance of:

1) The occlusion - bite has a key role in TMJ structural difficulty.
2) Jay to jaw bony relationships - structure alignment and its
importance to health.
3) The internal function of the TMJ disc and components.
4) The TMJ ligaments - the stylomandibular and stylohyoid in
particular.
5) The TMJ neuromuscular system.

Recently John F. Barnes, PT, among others, has brought to the medical consciousness the importance of fascia. This tissue connects us from head to toe and can exert a pressure of approximately 2,000 pounds per square inch. When the fascia is traumatized from a physical cause, a tremendous force may be encased in the physical body by the fascia! system causing pain symptoms.

As we examine the TMJ hard structures, not only are there many
fascia connections within the direct TMJ structures, but there are
connections throughout the body that may strain these structures.
Thus a trauma to one part of the body may affect another part.
Clinically we may see this after a car accident where the patient has
whiplash. Because of the strain in the back and/or neck, the fascial
sheaths traverse into the head and jaw area causing symptoms. Even though there may not have been a direct blow to the TMJ structures,they may have been adversely affected due to strain in another part ofthe body.

In examining a TMJ patient, proper diagnosis is necessary before
treatment commences. The physician, dentist or therapist should be
able to feel a strain pattern in the TMJ tissues that also may be
affected by problems outside the TMJ area. Because the fascia plays
such a critical role in health and disease, we recommend specific
treatment techniques to ease the sprain and strain on the TMJ soft
tissues. The following key areas are important for proper diagnosis
and treatment:

1) The temporo-occipital area just posterior to the TMJ - this area is
chronically affected and is especially important in an accident case.
The final occlusal position is critically affected by the tension in this
area.

2) The mandibular area - Because of the pathologic strain patterns in
the soft tissues, a calm, unstrained final occlusal position cannot be
attained until this area is treated successfully.

3) The maxillary area - The upper half of the TMJ complex may be
involved. In the same manner, the maxillary area must be treated
and unstrained for proper TMJ health.

4) The hyoid area - Often a forgotten area for treatment, suprahyoid
area will strain the mandible in a pathologic direction. This area must
be clear for TMJ tranquility.

5) The TMJ complex - The disc itself is attached by soft tissue, and
strain patterns that are present may create an abnormal functioning
disc. Every attempt to non-surgically correct this situation should be made by the therapist.

In summary, the TMJ complex consists of more than teeth, jawbones and muscles. The fascia! soft tissue component can exert tremendous pressures on the TMJ structures causing pain and other symptoms. With proper diagnosis and treatment of the fascial soft tissue structures, one can greatly enhance and expedite TMJ therapy.

Dr. Gillespie receives referrals from across the nation at my Myofascial Release Treatment Center in Paoli, Pa., from physicians, dentists and therapists for the resolution of complex head, neck and TMJ problems in adults and pediatrics. A primary purpose of this organization is to encourage research on oro-facial, craniomandibular, cervical pain and temporomandibular joint disorders.

Dr. Gillespie and I have also just designed a new seminar, "Myofascial Release and TMJ Therapy" that he will be teaching nationwide starting this fall. Dr. Gillespie has the most talented, sensitive hands of anyone I've ever encountered and is highly skilled in delicate TMJ and myofascial release techniques.

I would like to take this opportunity to share with you a myofascial
mandibular technique that he and I use quite regularly with consistent effectiveness. The myofascial mandibular technique that I am about to describe can be effective in reduction pain and strain and restoring the range of motion and critical balance in the temporo-mandibular area. When we are hurt, scared or angry, or when we are under sustained stress, our primitive responses tend to increase the tone of our masseters, our temporalis and our pterygoid musculature. This tightness can become a conditioned response over time. Occlusal discrepancies, upper cervical dysfunctions and/or forward head posture over time also create increased tone or spasm in those areas.

Trauma, inflammatory processes, or this increased stress can cause
the above-mentioned fascia! system to shorten and become restricted, acting like a "straight jacket" compressing and imbalancing the TMJ mechanism.

So even though this technique has the label "mandibular," please also consider using this technique for mandibular pain and dysfunction, headaches and with anyone that has been experiencing pain or stress anywhere in their body.

Myofascial Mandibular Technique

Phase I - Compression
Lie your patient on their back or, if you are a dentist, tilt your dental
chair back as far as it will go. Make your patient as comfortable and
as well supported as possible with pillows under their knees. If they
have forward head position, place a pillow or two under their head.
You should sit at the head of the table and slide your finger around the angle of the mandible until you feel the anti-gonial notches. Slip one or two fingers into each notch as a handhold.
If the patient has a TMJ appliance, he or she should wear this
appliance during treatment. In some TMJ patients, the retro-discal
tissue can be as hot and tender as a boil, so if the cephalad pressure causes discomfort or the patient experiences locking problems due to menisci dysfunction, do not perform these techniques. However, most patients will tolerate this well.

Phase II - Decompression
After slowly and gently removing your pressure, place your hands flat
on the sides of the face just in front of the ears over the TMJ
mechanism with your fingertips pointing towards the patient's feet.
Once your hand placement is correct, apply gentle medial pressure so that your hands don't slip on the skin.
Next apply gentle pressure with your hands towards the feet (caudad). You will quickly feel the resistance of the first fascia! barrier. In other words, the motion will come to a complete stop. Do not try to force through the fascia! barrieri it can't be done.

One of the keys to effective Myofascial Release is gentle but sustained pressure against the barrier.
Eventually, you will feel a softening like taffy stretching and then
motion moving your hands to the next barrier. Most of our patients
have multiple barriers that have developed over time, so it is wise to give the decompression phase at least three to four minutes or longer without letting go of the pressure. Repeat the technique over a series of treatments.

Always remove your pressure slowly to avoid a rebound effect of the musculature. It is usually advantageous to precede treatment with appropriate modalities and then give the patient proper instructions in head and general body posture and mechanics, coupled with an individually tailored exercise program to maintain and maximize the increased range of motion you have just accomplished for them.

Listed below are contraindications for myofascial release. These are
very straightforward and as long as these precautions are followed,
you will never injure your patient.

Do not hesitate to use Myofascial Release anywhere on the body.
These techniques are safe, easily accepted by your patients and they will see and experience immediate improvement in range of motion, as well as decreased spasm and pain.

Please consider adding these techniques to your treatment repertoire. I think you and your patients will be very pleased with the results.

Unhealed fractures, osteoporosis, tumors, recent surgical repair,
inflammatory processes on a patient who is medically unstable.
The above description of this technique is designed to give you a good
starting point.

Myofascial Release: Athletic, Sport, and Dancing Injuries

I graduated from the University of Pennsylvania in 1960 as a physical therapist. Back in those days, massage
therapy was part of physical therapy education. When I first graduated, every referral I got from a physician said the same thing, "hot packs, ultrasound, massage hot packs, ultrasound, massage." I just remembered moving the ultrasound over somebody's back and thinking: "I went to college for this?" I think I got better
results from the ultrasound on the days they forgot to plug it in!
Because I'd developed great respect for massage therapy during my education, I decided to become a licensed massage therapist. That decision turned out to be one of the best in my life.

Muscles, Fascia, and Pain: Myofascial Restrictions and the Body

Myofascial release combined with massage is wonderful; each enhancing the effectiveness of the other. Traditional stretching only releases about 20 percent of the fascial system. It
doesn't release the other 80 percent-the collagenous barrier of the fascia! system-which, in the past, has been ignored. We were taught that the fascial system covers the muscles-and it certainly does-but the fascial system also surrounds and profoundly influences every muscle, every fascicle, and every microfibril down to a celllular level. Fascia tightens down from trauma and shortens the muscle. A shortened muscle loses power and goes into spasm, resulting in pain. But it doesn't end there. Myofascial restrictions also
produce crushing pressure on pain sensitive structures, and that pressure compresses the circulatory system; the lymphatic system that lies within the myofascial component. This crushing pressure of the restricted area pulls the osseous structures too close together, jamming the facet joints and bulging discs, putting pressure
on adjacent nerves. This creates spasm, pain, and dysfunction. Osseous structures are being pulled out of alignment. Now the body is being pulled into a struggle within itself and limits optimal performance. And as we all know, optimal performance is crucial to athletes training and competing in their chosen sport.

How Fascial Manipulation Helps Athletes

Myofascial release, in combination with massage therapy, is ideal to use very gently prior to any athletic endeavor or performance. This will help enhance their performance, add to the fluidity of their motions and prevent possible injury. During the game or performance, if an injury has occurred and you happen to be on
the health care team attending to the athlete, massage therapy in its lighter forms can help assist the person get back on the field and/or help decrease the pain associated with the injury. Obviously, do not place pressure into the fascial system where there have been recent fractures, dislocations, concussions and/or bleeding. Fascial restrictions can create enormous pressure on pain sensitive structures, resulting in what I call a "straitjacket." All physical therapy modalities and various forms of massage, energy work, body work, manipulation, and mobilization release a portion of the fascial system. Unfortunately, none are as effective at
releasing the "straitjacket myofascial release. This is why it is so important to incorporate myofascial release your practice to resolve the totality of the problem.

Myofascial Stretching for Athletes

Myofascial stretching is very different than traditional stretching and all its various manifestations. Traditional stretching only releases about 20 percent of the fascial system or myofascial complex. I am sure you have all done some stretching for yourself over the years, and you may have noticed some progress.
But then you probably noticed that the progress stopped. No matter how long or hard you stretched, it just didn't seem to improve. This is the problem with the proverbial straitjacket (as well as the fact that myofascial restrictions do not show up on any standard testing}.

Myofascial Unwinding: A How- To

I would like to describe a myofascial stretch to do on yourself focusing on the hamstring/gastrocnemius area. Stand in front of a stable table or a large rock, something that won't move. Bend forward at the waist to approximately 90 degrees and place your elbows and forearms on the solid surface making sure to
support your back. Have your feet spread open comfortably and now imagine you have a string, lifting your trunk off your pelvis. From your hips begin to hinge forward until you feel resistance throughout the posterior aspect of your lower extremities. Instead of forcing into it, at this point, lift towards
the ceiling until you feel resistance again. Wait here and eventually you will begin to feel a tingle or pins and needles effect. You will eventually feel a softening and a new range of motion. Move into this gently. As you do this, you may also notice that your body may begin to move spontaneously. Allow this to occur. This is what we
call myofascial unwinding. This particular technique can be extremely helpful in elongating the posterior aspect
of the lower extremities and the lumbosacral areas. The key is timing, attempt to hold for five minutes longer
and always release the pressure very slowly.
