So today I did my other job: support for my lovely wife. As Regent of her DAR chapter she had. Barbeque for her chapter. Well let’s be more specific. I made my famous Beef Brisket. Low and slow, 20 pounds. Then I also made Jaffe Ribs and chicken sauce and of course ribs and chicken. Hamburgers 3 ways and hot dogs. Anything for her.

We now see 50-60 people a week, and average 6-10 new patients every week. So that means if I can get a part time PA or ARNP we could increase new patients to 10-30 per week.

What is more impressive though is that we have 3-5 patients every week who are well enough they go to a return as needed basis. Summit Pain that percent is 0.01%. Virtuous that number is 0.001%. Yet our percent is 5-7%. We get patients better, that is our goal, not turning patients in to monthly cash machines.

The practice keeps growing. Over 40% of patients are seen for fewer than 5 visits before their pain is controlled and they only return to say hello. Some have long standing issues and we get to know one another over time, hopefully achieving some level of control for them at the same time.

As the practice grows so do the needs of the practice. I have been looking hard for an ARNP or a PA-C. I will train. I have not yet paid myself but that is okay. I am looking to grow the practice more than I am looking to fatten my wallet. We are booking new patients four weeks out.

I am getting busier and busier. That’s what happens when you turn patients in to family. Even a simple follow up visit involves discussion of a brisket recipe. I have heard most doctors do procedures and the patient schedules their return visit. Imagine having just just one injection and being told to call us if the pain returns. Technique is everything!

STEM CELL THERAPY
also known as Regenerative therapy.
As a patient suffering from arthritis, back or neck pain, or spine issues, you will see many “free dinners” or other advertising gimmicks to get you in the seats. Then you get this wonderful presentation that promises to cure almost anything and maybe make you a young man again.

So let me tell you some truths. There are a few ways to harvest “stem cells”. The easiest is to draw blood and spin it and concentrate the platelet and plasma portion. This is known as Platelet Rich Plasma. You can than take the discard and extract an enzyme and protein rich portion, about 10% of the volume you began with. More invasive you can take cells from a bone marrow biopsy. This is very rich in juvenile cells. Lastly you can take what is called “Omental Fat” or fat cells from inside your abdomen. This is then processed.

The success of Regenerative Therapy (RT) is dependent on a few things and I will also give you the percentage weight of the importance of those things.
#1) The most important factor is experience. This counts for 35% of the chance of success of RT. Choosing someone like Dr. Jaffe with over 25 years experience in RT is important. But there are other factors.
#2) Does the provider have experience performing joint injections? And are they using Ultrasound or Fluoroscopy to do the injection? This is 30% of the success. I did PRP for a dentist’s son years ago who had a partial tear of his meniscus. Five months later he was back as Captain of his soccer team. Later I injected PRP for the father who thought he was going to have to retire. He could not sit or stand without pain. One injection in to his hip joint and he is still going strong ten years later. Both injections were done done with a combination of ultrasound and fluoroscopy.
#3) Your age and the severity of the problem. This is 25% by weight. Let’s be honest, I can inject 0.1 milliliters of stem cells in a 12 year old and get success. Be sure you go to a doctor (and chiropractors are not supposed to give injections in the state of Arizona) who will be honest with you.
4 #) The last 10% is the unknown factor. Call it God’s will or luck or faith or prayer but some who should not heal, do!

Just watched a show about the “Pill Mill” time in Florida before the E-Force. Fortunately I was ahead of it all. My practice had a relationship with the pharmacists and the Sheriffs and Police. The pharmacies could call me any time, the officers day or night. We did not give prescriptions first visit. And now we know even more that opiates do not really treat pain. Fortunately there are many procedures that can treat pain. It is why we take plenty of time the first visit to be sure of the best course of treatment. I am a third generation physician and treat every patient like family.
