Don’t put your health on pause. With summer here, it’s the perfect time for teens and young adults to schedule a well visit (or check-up) with a primary care provider (P*P).

Your P*P is the provider that you go to for all your regular check-ups. They can offer a safe space to talk about:

- Health screenings and vaccines you may need
- Self-care topics such as eating healthy and staying active
- Mental health, including stress, depression or anxiety
- Sexual health, alcohol/drug use and more!

As an HPSM member, you get these well visits at no cost. Plus, HPSM Medi-Cal members who are 12-21 years old and assigned to a San Mateo Medical Center clinic can get a $25 gift card for going to their well visit!

Do you have diabetes? Understanding yourdiabetes ABCs can help you manage your diabetes:

A = A1C (blood sugar) test

C = Cholesterol

It can also lower your risk of diabetes-related complications like heart disease, stroke, kidney disease and vision loss.

Your mental health matters — and help is available.

Seeking care for your mental health is just as important as visiting your primary care provider (P*P) when you’re sick or something hurts. If you notice any changes in your mood that affect your ability to take care of yourself, participation in daily activities or your relationships, it’s a good time to ask for mental health care:

• Talk with your primary care provider (P*P) about how you’re feeling. They may have recommendations to help you feel better.
• Call the Behavioral Health and Recovery Services (BHRS) ACCESS Call Center at 1-800-686-0101 (TDD: 1-800-943-2833) to be connected with a mental health provider.
• Your HPSM benefits cover mental health services at no cost, including talk therapy and medicines.

anxiety.

If you or a loved one is struggling emotionally while pregnant or after giving birth:

- Talk to your primary care provider(P*P) or prenatal provider about how you are feeling. They may have recommendations to help you feel better.
