If you are a North Carolina teacher, state employee, or covered family member with the NC State Health Plan, you may now select Casey H. Hall, FNP-C at Roots & Ridge Health as your Primary Care Provider.

If you would like to make Roots & Ridge Health your primary care office, you can log in to your NC State Health Plan member account and update your P*P selection to:
