A meta-analysis found unintentional weight loss associated with 49% higher mortality risk in patients with comorbidities, and 81% higher in older adults. The body sends that signal weeks before any other symptom appears.

The problem is that most care models aren't built to catch it. Patients are weighed at a visit, then not again for months. A condition shifting weight in January may not reach a clinician until March.

Most practices are already doing the work that qualifies for CCM reimbursement, but they’re just not billing for it.

Care coordination, follow-ups, and ongoing patient management have always been part of good care. What’s changed is that Medicare now pays for it. Yet adoption is still low, with only a small percentage of eligible patients enrolled.
