Our new Hospital to Home (H2H) Transitional Care Program is designed to bridge the gap between hospital discharge and recovery by providing a primary care visit within 24–48 hours after discharge, with same-day visits available for high-risk patients.

To help seniors age in place safely while reducing unnecessary hospital readmissions through compassionate, coordinated, mobile healthcare.

Our H2H Program Includes:
