If you haven’t already, while you're in the payroll files:

βœ… Review employment contracts
βœ… Check above-award staff are handled consistently
βœ… Update position descriptions
βœ… Confirm mandatory training records
βœ… Check immunisation and certification currencies
βœ… Tidy staff files before accreditation asks for them in coming months or years!

Bulk billing rates are up & out-of-pocket costs are up.
At the same time.

It sounds like an error in the data. It isn't.

The incentive structure is all-or-nothing. Clinics that go fully bulk billing get removed from the out-of-pocket average entirely. The clinics that stay mixed or private receive no additional incentive β€” so as their costs climb, their fees follow.

Two trends moving the same direction, for completely different reasons.

If you're mixed billing and feeling squeezed, you're not doing anything wrong. The maths simply isn't in your favour.

The short version: consent is moving to a digital framework, minus the DB4 form. How you obtain it, and how you record it, both matter now more than ever.

What that means practically:

β†’ Your paper process may no longer be sufficient
β†’ Your software vendor's readiness is now your problem
β†’ Record keeping obligations have shifted
β†’ Reception needs a script, not a guess

Most practices we speak to think they're compliant because their software provider said so. Worth confirming that yourself.

Six questions to ask before your next fee review.

1. When did you last calculate your actual cost per consult?
2. Have wages, rent, software and insurance been factored in β€” or just Medicare indexation?
3. Do you know what comparable clinics in your area charge?
4. Is your team confident explaining the fee, or apologising for it?
5. Are your fees consistent across doctors, or wildly different?
6. When patients ask "why has this gone up?" β€” does everyone give the same answer?

If you're unsure on more than two, your fees probably aren't reflecting your real cost base.

Following on from our recent podcast, should doctors discuss their fees with patients?

Ami says no. Zac says yes. We couldn't agree on this one.

Ami's position: doctors should focus on medicine. Trained admin staff handle financial conversations β€” it's smoother for the patient and protects the consult.

Zac's position: if doctors avoid the fee conversation, it quietly undermines private billing. Confidence in your pricing has to start with the person setting it.

We genuinely had to agree to disagree on this one. So we're asking you.
