A landmark study reveals hormone replacement therapy could lower dementia risk for women, but the benefits depend heavily on starting treatment during a crucial midlife window.

A groundbreaking study of over 183,000 postmenopausal women in the UK Biobank has revealed that hormone replacement therapy (HRT) could help defend against dementia, but only if timed correctly. Published in Alzheimer's & Dementia, the research indicates that starting HRT between the ages of 46 and 56 offers the strongest cognitive protection, leading to a substantial drop in dementia and Alzheimer's risks. Crucially, the benefits are highly individualized; women who underwent surgical menopause, carried the high-risk APOE4 gene, or had shorter natural estrogen exposure saw the most pronounced brain-health improvements.

While these findings offer fresh optimism, experts caution that the observational study does not definitively prove causation. Historically, concerns over hormone therapy risks have kept many women from exploring treatment, but this research reinforces a vital 'critical window' theory: early estrogen intervention may protect the brain before age-related decline sets in. As the medical community moves toward personalized menopause care, these insights provide crucial guidance for women and doctors discussing how to best mitigate cognitive decline.

Ovaries have a second life after menopause as immune organs.

Scientists once viewed postmenopausal ovaries as largely inactive organs that had finished their reproductive work. But a new study suggests they may not simply shut down. Instead, they could begin behaving more like part of the immune system.

Researchers examined ovaries from young, reproductively aging and postreproductive mice. As expected, genes involved in egg development, hormone production and reproduction became less active with age.

But something else took their place.

Postreproductive ovaries developed an “immune-like” molecular profile. They contained increased numbers of T cells, macrophages and other immune cells, while genes linked to inflammation and immune signaling became more active.

The ovaries also showed signs that they might produce inflammatory molecules capable of traveling beyond the organ and affecting other parts of the body.

That could mean ovaries continue communicating with the body long after ovulation ends. Researchers speculate that this transformation may influence chronic inflammation and health during aging, although they do not yet know whether its effects are helpful, harmful or both.

There are early signs that human ovaries also remain active after menopause. In a separate preprint involving ovarian tissue from 28 women between ages 50 and 75, researchers found that the proteins produced by the organs continued to change with age.

However, the strongest evidence for an immune transformation currently comes from mice. Mice do not experience menopause in exactly the same way humans do, and the study included only three to four animals in each age group.

Researchers have also not confirmed that older ovaries actually release immune signals into the bloodstream or affect other organs.

The discovery therefore does not prove that ovaries become immune organs in women. But it challenges the long-held idea that they become useless after their reproductive role ends.
