Notice of Practice Closure

Please be advised that Maczko Chiropody & Orthotic Centre, located at 559 Exmouth Street, Sarnia, Ontario, will permanently close effective September 30, 2026.
• Patient Records: To obtain copies of your patient health records, please complete a Patient Request for Personal Health Data Form, available at the Maczko Chiropody office. To request the form, you may come into the office or call 519-337-9540 or email [email protected]. Once completed, bring the form to the office, and we will process your request.
• Records Storage: Details about the long-term storage location for patient records will be made available in the coming weeks.
• Ongoing Care: For future foot care needs, please complete a Consent Form authorizing the transfer of your personal health data to another chiropodist. Chelsea Charteris, B.Sc., D.Ch., and Allison Deery, B.A., D.Ch., will operate under the new clinic name Lambton Chiropody & Health Centre. You may contact them directly to book future appointments.
