When someone finally asks if s*x hurts, most women say yes and then nothing happens with the answer. No follow-up questions, no exam, no plan. Just a nod and a suggestion to try more l**e.

The follow-up questions are so important. Pain at the entrance and pain with deep pe*******on come from different structures and need different treatment. Burning that shows up the next day behaves differently than pain that stops the second you stop. Pain that started after a C-section is a different story than pain you've had since your first tampon at fourteen. We ask where, when, how long, and what else, because those four answers usually get us most of the way there.

At our clinic in Williamsburg, Brooklyn, we treat pain with s*x in clients from their teens through their eighties. A first appointment is an hour: history, an external and internal assessment, and manual work on the muscles and tissue we find involved.

A lot of women get sent for imaging, get a clean result, and get wrongly told to just work on their stress levels. The scan did its “job”. But it wasn’t looking for the thing that actually hurts.

Pelvic floor muscles don’t show up on an MRI the way a cyst or a fibroid does. When those muscles hold tension for months or years they get short, they get irritable, and they refer pain into the low back, the hips, the bladder, and the v***a. That pattern is completely findable. It’s just findable by a hand in an exam by someone trained to look for it, and it responds well once someone does.

A clean scan rules things out. It doesn’t explain what you came in for, and you don’t need a diagnosis on paper to deserve treatment. September is Pain Awareness Month. DM us “PAIN” and we’ll walk you through what a first appointment looks like.

We said we’d do a part two, so here it is! Same as last time…you do not need to apologize or feel weird about anything on this list.

Discharge that changes through the month…it’s supposed to. It shifts from thicker to thin and stretchy during ovulation and back again. Your cervix is doing its job. What is worth asking about is a change in smell, color, or itch, not the fact that it changes at all.

Your v***a not being symmetrical…almost nobody’s is. One side of the l***a is usually longer or larger, and that’s just anatomy, not a flaw.

Feeling nothing the first time you go looking for your pelvic floor…that’s where most people start! These are muscles you have never consciously used and can’t see, and not being able to find them on command at first is normal. It’s the reason we assess instead of handing you a list of exercises and saying good luck (that would be crazy).

Having somewhat of a smell down there…a va**na is not meant to be odorless. And it shouldn’t smell like perfume! A strong change in order is worth a conversation, but baseline smell is normal.

Being nervous at your first appointment…it’s new! It’s unfamiliar! You can stop at any point and ask us to explain anything before we do it. You have full control and we’re more than happy to walk you through things to make you more comfortable.

Passing gas during treatment…we’re working directly on muscles that hold it in! It happens constantly and is so normal.

Needing l**e at any age…this is NOT a verdict on your body or your relationship. Hormones, medication, stress, breastfeeding, and the menstrual cycle all change lubricatjon, and using l**e is nothing to be ashamed of, it’s just smart.

What would you put on this list?? I could go on and on about why I love each of these. When it comes to what really helps your pelvic floor, there are a few swaps and lifestyle considerations that I think are worth prioritizing.

But truly, the best work is free and comes in the form of breathing, moving your body, letting go of what doesn’t serve you, and being kind to yourself. Those things will always be priceless.

If s*x hurts, you have probably been told to use more l**e, have a glass of wine first, try to relax. That advice skips over the fact that pain with pe*******on is a physical symptom with physical causes, and it’s one of the most common reasons people come to us for pelvic floor therapy.

Pain with s*x has a clinical name: dyspareunia. It almost always traces back to something we can actually find. Often, the pelvic floor muscles are overactive and can’t lengthen enough for comfortable pe*******on. Sometimes it’s nerve sensitivity at the va**nal opening, called vestibulodynia, which burns or stings and is often missed. It could also be scar tissue changes from lower estrogen while breastfeeding.

At our clinic in Williamsburg, Brooklyn, pelvic floor therapy for painful s*x means hands-on work to release muscle tension, scar mobilization, nerve desensitization, and nervous system work so your body stops bracing before anything even starts.

Avoiding s*x is not a treatment plan, and neither is pushing through pain. If any of this sounds familiar, please come see us.

Let’s get real here for a sec.

We know that a lot of people’s barrier to starting pelvic floor therapy is the cost. And we know it’s not cheap. But when you put it into perspective, pelvic floor therapy costs less than a lot of things New Yorkers buy without blinking. And unlike buying liners on subscription, it actually fixes the problem, without racking up sneaky costs.

If you’ve been leaking since your last baby (or your first), that’s treatable. Most of our clients feel a real difference within a few appointments.
