Recent data indicates that any diabetic who has had a diabetic foot ulcer be monitored monthly to achieve the lowest rate of recurrence! See chart below.
A diabetic foot ulcer is unfortunately a life long risk factor after it has occurred. The skin and tissue that fills in the wound will NEVER achieve original strength and durability. Best guess estimates are around 80%. This is why the terminology has changed from healed to "in remission" due to the high rates of recurrence. One of many things that experienced podiatrists can do better than anyone else is see problems before they occur! A visual inspection provides a great deal of information to us. Is there a callus, a bruised callus( pre- ulcer), xerosis( very dry skin), a contracture of a toe or foot, a flat foot , high arch foot, etc. all which when combined with the physical exam can alert us to the area that will potentially ulcerate. Neuropathy and Vascular disease (numbness and poor circulation) are compounding factors that worsen outcomes. If you have had a diabetic foot ulcer, don't miss your follow ups. It could come back to bite you!

This sounds like a great idea, hopefully it explains beyond just Eastern Pa and the Jersey shore!

"The traditional healthcare landscape is undergoing a revolutionary shift as doctors look beyond the pharmacy counter to treat chronic illness. Pioneered by Pennsylvania’s Geisinger Health system, the Fresh Food Farmacy program treats food insecurity and Type 2 diabetes simultaneously by writing “prescriptions” for free, nutrient-dense foods. Instead of relying solely on expensive, lifelong pharmaceutical regimens, patients are provided with fresh produce, lean proteins, and whole grains tailored to their condition, alongside comprehensive nutritional coaching. This clinical approach recognizes that a lack of access to healthy food is a primary driver of metabolic disease, treating the root environmental cause rather than just managing symptoms with meds.

The clinical results of this “food as medicine” approach have been nothing short of transformative. Early data from the program showed that providing patients with fresh food and education lowered average HbA1c levels dramatically from 9.6 to 7.5–a clinical improvement that rivals and even exceeds the impact of standard, high-cost diabetes medications. This success has triggered a nationwide movement, with hundreds of doctors and major public healthcare institutions from New York City to California now launching produce prescription programs. By proving that investing in nutrition food can drastically reduce healthcare costs and combat diet-related illnesses, these programs are proving that a trip to the grocery aisle can be the most powerful medicine of all.

Encourage your doctor to participate in this Fresh Food Farmacy Program and all of the ways whole foods can help lower your blood sugar better than medications.
‼️HOWEVER, NEVER STOP TAKING YOUR MEDICATION WITHOUT CONSULTING YOUR DOCTOR FIRST‼️"

Plantar fibroma, not to be confused with plantar fasciitis, is a relatively common complaint. As described in the photo, it feels like a small firm lump which can be painful or non painful depending on location.
Some causes are listed below, but many patients present with none of these causes in their history.
Treatment is based on size, location and how painful they are when walking. There are some conservative treatments that can help with pain but definitive treatment usually requires surgical excision. This is usually delayed until some conservative treatments are tried as plantar fibromas have a very high recurrence rate, however they usually come back smaller and with less discomfort than the original lesion.

One of the treatments that I use is off label use of a medication called verapamil which is a high blood pressure medication. You may ask why a blood pressure medication? This medication is not given orally but is compounded into a gel or cream to be applied over the area where the fibroma is. There have been studies showing that this shrinks the plantar fibroma if used consistently over time and may preclude use of surgery to eliminate the problem. Cost and coverage by insurance companies is sometimes an issue with this option but if you want to avoid going under the knife, it may be worthwhile to try this before talking to a surgeon as cutting through the bottom of the foot results in slightly longer healing times just because of the nature of the thick skin there.
Other differential diagnosis that present in the foot similar to this are lipomas which are just fatty cysts, foreign bodies which can develop into a lump from the body trying to wall off the foreign body, callus, scars and other types of various cysts.

If you notice something like this in the bottom of your foot, call our office at 724 -632-6801.
