As we get older and live within the mid life, we find ourselves at the pinnacle of burden. Taking care of our aging parents all while raising our own children, paying bills and trying to maintain personal health and career trajectory.

Many young adults refrain from having kids as they build careers. I did not. I had children while being very poor and building my own career. I wanted them to experience the hardship and have direct experience with what Dad was doing.

After ten years in practice I don’t regret this decision. I cherish it. I am not the world’s greatest Dad. I am also not a world renowned famous surgeon. But I do both the best I can. Doing both has created a deep understanding on complex problems and allows extreme humility. I think the balance is beautiful.

I can’t tell you how proud I am to see my oldest daughter attack this life. She wants to serve and lead the next generation. She is learning how the hardest path is always the answer. The outcome will just take care of itself organically. The greatest goal to achieve is high character and community contribution.

There is a counterculture starting.

Many people are starting to realize discipline equals freedom. Everything we do in life requires the function of our body. To withstand hardship of any kind we must be secure with our heath. If we loose it all, as long as we have the strength within our mind and body….our soul can rebuild.

Fitness is about building the mid-cingulate cortex and a more ELASTIC heart. It’s not about cosmetics or gym culture. It’s about freedom later in life. It’s about leadership to your children. If you speak leadership and represent this position than be a walking resume.

Grab a 40 lb vest and an 80 lb Kettle and move your machine. 15 minutes back and forth no rest. It’s a Norwegian high interval training routine. Do this 5 days a week and research shows you will delay your aging. Plus it’s a quick and refreshing phase adjustment just in time to start work or put your kids to bed.

Nothing will ever compare to the time that is allocated for these experiences. It’s who we are and what we are made to do.

Football shaped me.

It’s football season. A great time of year. The boys of Fall are about to start their season.

I played this game for 18 years. The 80’s,90’s and early 2000. It was hard. Even harder to maintain good grades. During my college years, I faded away from football to concentrate on pre-med. Being an academic minded student athlete for 4 years was difficult. But, I learned that hard was a good thing. I miss those days. I miss the camaraderie, the traveling to different universities and warming up with the drum line.

Over the years this game kept me accountable. It gave me purpose. It taught me “all for one” and “one for all.” When I realized it was over, I entered into a new very challenging endeavor. It was time to challenge my mind and spirit not just my athletic physicality. I also needed to feel a sense of purpose with a new team. This new team was the United States Navy.

I never stopped choosing the hard road. I wanted to build a team at home. So I did. 6 kids and a 21 year marriage. Being single or engaged and committing one self to a hard challenge is actually easy. So I decided to add 6 kids to the mix. Haha.

This game is as American as it gets for our young men. If I was to give a young football player advice I would say this. Love this sport whole hardily. But don’t let it define you. Use it as a tool to help create an iron mindset of pure will and determination. Use this sport to teach you life.
Don’t bask in the glory days of this game. Let it propel you to more and tougher challenges.

Elective orthopedic surgery is the very last resort

I dislocated my shoulder years ago snowboarding. I tore my labrum with a Reverse Bankart fracture.

There was a very small study out of Japan that I followed for non-operative management. I also dedicated myself to a functional shoulder stabilizing rehab protocol and PRP. It healed and I have zero issues with my shoulder.

Trauma is a different story. However, fractures don’t have to happen when a traumatic event occurs. If you have great bone structure, mobility and good muscle support, your bone is protected to a certain degree.

Functional MSK medicine is legitimate.

The pillars.

-You must maintain muscle strength your entire life. Resistance training with heavy weight is key.

-You also must increase functional mobility and maintain muscle “Power” as much as possible.
This is more than just stretching. It’s called perturbation which is loading the body while encountering an unforeseen force.

-High intense interval training increases your bodies ability to use oxygen for more efficient enzymatic processes. This increases Vo2 max.
Studies prove high Vo2 max in late life decreases morbidity and mortality by 60 percent.

-zone 2 cardiovascular (aerobic) exercise helps support mitochondria function and helps increase your metabolic rate for more efficient use of calories. It also helps support blood pressure and vascular wall inflammation.

-Misogi. Doing a physical/mental challenge every year is great for mental health. It provides a goal to drive discipline. Humans must strive for adventure and purpose. This should be done with a very close group of people you love. The Japanese developed this ideology as to push the human spirit and maximize growth through stress and failure.

-Rest and recovery is an absolute must as to reset the sympathetic and parasympathetic nervous system. Some great evidence supports cold plunging, sauna, red light therapy, laser therapy, evening walks and of course sleep. If you task your mind and body with high stress, you must allow the brain to detoxify. This is REM sleep.

-Hormesis is a term where the body turns on its scavenger system as to help cleanse the body of dying cells during periods of stress. This can be done with intermittent fasting. Or periods of high stress to the body. Encountering high environmental stress by doing physical exercise in very cold or hot weather can help support Hormesis.

-Nutrition. This basically comes down to proper micro and macro nutrients that support your immune system and MSK system. Proper enzymes and co-enzymes require plentiful replenishment of nutrients used with these molecular reactions. Stay away from any man made preservative, pesticides, artificial sweeteners, hormones, antibiotics….

-hormone balance. Be careful using exogenous hormone replacement before you do all the above. Exogenous hormone replacement is good only if conducted by a well trained physician that is able to understand the foundation of why your hormone levels are low. Be careful, once you use artificial hormones, your body won’t produce them anymore. So why not try to ramp up your natural process first?

Instead of giving some doctor a five star review give yourself a 5 star review with your own health investment.

Ps. Surgical procedures return the highest reimbursements to your community hospitals and doctors. Human nature will create bias as the numerical value of money is the top metric. While surgery could be needed, make sure you get well evidence based information regarding the whole picture. Most orthopedic surgeons see 40 patients in clinic. This usually equates to 5 of those patients requiring surgery. Meaning 35 of those patients are in pain and surgery is not the current answer. If the provider does not provide adequate time and effort discussing the deep foundation of your pain with non-surgical options then I would be concerned about their bias.

I am going take a slight turn into prevention.

A lot of orthopedic surgeons and physicians are starting to speak out about longevity health and prevention. A lot of Doctors are creating online content as to share their practice with prospective patients. The physician world is entering the online matrix as the average person spends 5 hours on their screens.

For years I was reluctant to post about my career and share information. Most doctors still choose to keep their practice within the doors of their clinic and OR. Initially, I thought keeping it quiet and personal was the most professional way. However, health literacy is of utmost important in our current state of affairs. The Covid 19 pandemic completely changed our health care climate and exposed some huge issues. Health care is now a huge online presence and resource and patients are ever more confused.

Medicine is shifting. We have gotten into such a crises, we have no choice. Chronic disease and advanced age are creating huge issues with access to care and our overall quality of life. It’s a huge burden that creates country wide stress. Most of our health care institutions are still only very reactive and play a huge chess game within this business.

Despite our advanced care with trauma and lifesaving procedures, we are one of the worst advanced countries in the world when it comes to overall health. I can tell you the financial output for chronic disease and poor health later in life is going to spill over as the years stack up. The stress of health care costs will continue to be placed on patients and providers. Hospital systems, federal government and insurance companies will not compromise their bottom line.

There really is no point arguing this with political affiliation or protesting. It’s to huge.

The power of health literacy starts with YOU!

As a society we must start exercising and conditioning our MSK system. Our society must become wiser when it comes to our individual health. We must start investing in safe preventative measures and learn where to go for competent medical advice. Being able read medical literature correctly is extremely difficult. Especially with AI. There are huge amounts of studies that are extremely biased. This confirmation bias can only really be deciphered by a non-biased scientist and or an experienced physician that can really dissect the study.

This is the purpose for my online account. I post to give my friends and family non-biased information and to help improve your overall health literacy. I want to help all of you navigate this crazy world of health care. There is zero incentive to market or prove my relevance. I have been in practice 10 years. I have completed every advanced training I can. I have passed every board exam and practice with up to date board certification. I am NOT perfect. But, I have nothing to prove and I am absolutely not trying to show face as systems and doctors compete for the market share of patients. I have competed all of my life. It wasn’t easy to get here. I am 47 years old and at the 50 yard line. I have completed the race. I am very disciplined with all aspects of my life including financial literacy. I could stop practicing if I wanted and retire. If I see 5 patients or 30 it doesn’t matter to me. I’ll do my best with whomever comes through the door. I choose to continue because I care about my country, community, friends and family. Most importantly I serve my God and nothing else.

What kind of surgeon should you see?

It’s a very tough decision in today’s climate.
There are ways to look up a surgeon’s track record regarding complications, re-admission rates and outcomes. But, patient testimonials and word of mouth are the most consistent with getting a competent provider to help solve your health problem. Be careful with Marketing schemes.

Surgeons are also athletes. Meaning they need high reps and consistent growth. Low volume surgeons likely choose easier cases or are early in their career. This does not mean they are not skilled but just early on in their experience. Like a rookie athlete. Very high volume surgeons are usually very experienced and highly skilled. Over the years they have seen it all and built a great reputation. However, they are human and can get overwhelmed and start cutting corners to stay ahead of their high production.

The sweet spot is a moderately high volume surgeon that sees their practice as a marathon. This surgeon or provider knows their limitations/boundaries and realizes there needs to be a buffer zone with his/her time to allow deep meticulous work. And just like athletes, surgeons can burn out mentally and physically. Production and Money never enter his/her brain as the start or end point. This is actually rare. It’s all about the craftsmanship and safety…….along with the organic nature of the healing arts that drives them. They also practice what they preach.

Being a Surgeon and raising 6 children sometimes can feel pretty impossible.

I hope being good at one makes me better at the other. Many times I haven’t been home and when home not truly present. I struggle with this. But as I mature into mid life, the experience of doing both has increased my thermostat for stress. Everyday there is a new problem to solve and that is welcomed.

I chose the front lines and felt part of parenting was absence for a great cause with great contribution.

My daughter Jojo is thinking about military medicine as she heads into her last year at West Point. She has seen me struggle with high burden and has decided to follow the same hard path.

My family makes me a better surgeon.
And watching Jojo obtain grit makes it all worth it.

Always choose the hardest path. There is gold at the end.
