Order of operations matter, Rule out catastrophe first, then assess function!
Immediate On Field Athletic Trainer Assessment After Line Drive Contact

1. Scene / responsiveness
* Is the athlete conscious?
* Any loss of consciousness?
* Are they alert, tracking, answering questions?
* Obvious distress or guarding?
2. Primary survey (ABCs)
* Airway patent?
* Breathing normal?
* Circulation adequate?
* Massive bleeding?

3. Immediate cervical spine consideration
* Mechanism concern for neck involvement?
* Midline neck pain?
* Neurologic symptoms?
* If concern → minimize movement.
Head / Concussion Screen (highest priority if head/face contact)
4. Concussion red flags
* LOC
* Confusion
* Delayed responses
* Blank stare
* Memory loss
* Headache
* Dizziness
* Balance deficits
* Nausea/vomiting
* Sensitivity to light/noise

5. Cranial nerve quick screen
* Eyes tracking equally?
* Pupils equal/reactive?
* Facial symmetry?
* Speech normal?
* Hearing intact?

6. Eye / orbital injury
* Double vision?
* Blurry vision?
* Pain with eye movement?
* Orbital step off deformity?
* Visual field deficits?

Face / Skull Assessment

7. Open wound / bleeding control
* Laceration?
* Scalp wound?
* Facial bleeding?
* Control bleeding and assess depth.

8. Facial fracture screen
* Nose deformity
* Jaw pain/malocclusion (“does your bite feel normal?”)
* Zygomatic tenderness
* Crepitus
* Tooth injury

Neurologic / Functional Screen

9. Neurovascular status
* Sensation intact?
* Strength equal?
* Finger squeeze
* Gross motor control

10. Coordination / balance
* Can they stand?
* Tandem balance
* Gait changes

This is typically all done within 5-7min or less! Then the secondary assessment begins!

Wild World Cup injury
This one’s wild because it looks like “he just got kicked in the calf,” but the mechanism can create a huge bending force through the lower leg.

A direct kick or trip to the back of the calf while the foot is planted can trap the lower leg while the body keeps moving forward. That creates rotational and bending load through the shin and can result in a tibial shaft fracture. In high level sport, if the fracture is displaced, the medical team’s priority is stabilization, getting the athlete off the pitch quickly, imaging, and then often surgical fixation with an intramedullary rod through the tibia to restore alignment and allow healing.

This is just so unnecessary. I read a fair amount of literature on various manual therapies and never once does it say that you need to peel the skin off you patients.

It also feeds back into the idea of just cause you’re certified in something doesn’t equate to true knowledge or application.
Certifications are base levels.
True understanding and application is mastery

There’s so much research out there, my entire algorithm is coaches/clinicians dropping bars of research and how to use it.
It’s on the consumer to know how it applies within their practice, you just have to be willing to utilize it

Fracture management from an athletic trainer.
Things can go sideways quick during a game, and when they do, here’s your checklist
Pulse. Sensation. Toe movement. Cap refill. Skin integrity. Pain response.
And the key everyone forgets… reassess everything after splinting.
Protect the injury. Protect the tissue. Protect the athlete.
