You can be awake and not conscious.
Dr. Jerome Puryear, MD, is an obesity medicine physician and an interventional radiologist. He spent years on night call. It took another sleep physician to tell him he had obstructive sleep apnea.

He knew what exhaustion looked like in everyone else. He missed it in himself.

That is the trap in shift work. You lose the baseline. Every morning feels like the last one, so nothing feels wrong.

On the episode he tells about driving in around 5 or 6 AM, rested, headed down I-75. The 18-wheeler ahead of him swerved hard and left the road, then corrected and came back. It happened again. By the third time they had reached the rest area, and the rest area was full. Trucks were parked along the side of the road because there was nowhere else to put them. The 18-wheeler hit one of them. Debris came across his lane and he had to swerve clear.

There is a common cause for this - drivers cycling through microsleeps.

Eyes open. Hands on the wheel. Not processing the road.

Awake, but you not conscious.

If you work nights, rotate shifts, or drive home after a twelve, this one is for you.

Is there a universal caffeine cutoff before sleep?
Dr. Jerome Puryear, MD, covers this and so much more in our latest episode about sleep.

Why that "no caffeine after 2 PM rule" applies to you and not your coworker
How tea, candy and other food products affect sleep
Even how your workout affects your sleep

Scrolling in bed is not a willpower problem.

Screen light suppresses melatonin, the hormone that tells your body it is time to sleep. The TV counts too. Your brain keeps processing it.

Dr. Jerome Puryear, MD, breaks it down. Full episode is live now on YouTube and anywhere you consume podcasts.

You cut sleep first when life gets busy. It's the one thing you can't actually cut.

This week on Dialed In Health, we sit down with Dr. Jerome Puryear, MD — an obesity medicine physician who spent years on night call before he diagnosed his own sleep apnea. He wrote the book on sleep deprivation because he lived it.

We get into it:

→ How many hours you actually need (and why the "everyone needs 8" line is lazy)
→ Caffeine timing — and the hidden sources nobody counts
→ Insomnia vs. sleep apnea, and when it stops being "just snoring"
→ Shift work: what to do when your job fights your body clock
→ Straight talk on melatonin, prescription sleep meds, CBD and THC
→ Sleep trackers and AI sleep tools - worth it, or hype?

New episode drops this week. Turn on notifications so you don't miss it. 🌙

"It's neuropathy, nothing you can do."
Too many people have been told exactly that.

So we asked Dr. Jordan Stenzel what actually helps.

The 5 supplements he sees used most for nerve health: L-arginine, B12, omega-3, vitamin D, and superoxide dismutase, and what each one does.
