Meet Jordan Smith, PA-C 👋

Jordan Smith is a Physician Assistant with 12 years of clinical experience and a strong passion for building long-term, meaningful relationships with patients.

With over seven years in primary care and four years in the Medical ICU at Cleveland Clinic, Jordan brings a thoughtful, well-rounded approach to patient care rooted in both prevention and clinical excellence.

With a background in Exercise Physiology, Jordan has a strong interest in metabolic health and whole-person care. At Maple Health Direct Primary Care, Jordan is committed to providing accessible, relationship-based care where patients feel heard, supported, and empowered.
