It's important to be prepared for an emergency. Montgomery County has a downloadable File of Life form that makes it easy to have important medical information available for first responders.

Downloading it, completing it, and hanging it on the refrigerator are just the first steps -- please be sure to UPDATE it with any changes. It's a good idea to review it after each doctor visit to see if you need to make changes to the medication list or add any new diagnosis that's been given.

Many people have multiple physicians so it's a good idea to take it with you to each doctor appointment also for an easy reminder of medical history information and medications.
