Credentialing is the silent revenue killer. A provider seeing patients before the payer finishes enrollment means weeks of held claims. Start enrollment the day the contract is signed, track every payer's status weekly, and never let a start date outrun the paperwork.

Patient balances are the slowest money in the practice. Statements that go out late, or only once, don't get paid. Bill the patient the same week as the payer, send a second notice at 30 days, and offer a card-on-file option. Speed and repetition collect.

Court ruling worth knowing: patients do not have a legal right to ambient AI recordings of their visits.

In Raphael v. Mantei, a Washington judge ruled that the audio an AI scribe captures (in this case, Nuance DAX during a telehealth visit) is exempt from disclosure. The reasoning: the recording serves an "administrative purpose," the same as dictation or handwritten notes. Only the physician's final clinical note counts as the record of care.

What this means for practices using AI scribes:
- The draft audio is not part of the medical record. The final note is.
- This is one of the first rulings in the country on AI scribe technology, an early precedent for compliance teams.
- It is Washington law only. It does not settle HIPAA's right of access or state consent rules, which still vary.

If your practice is adopting ambient documentation, your policies should spell out what the recording is for, who can access it, and how long it is kept. Need a second set of eyes on that? That is what we do.

Prior auth denials are usually documentation denials. The service was fine; the chart didn't say why. If your auth requests quote the payer's own medical-necessity language back at them, approvals go up. Write to the policy, not to the doctor.

Work your denials while they're young. A denial worked within a week has a real chance. The same denial at 90 days is nearly a write-off. Set a rule: no denial sits untouched more than 5 business days.

Most eligibility denials aren't payer mistakes. They're stale data: a patient changed plans, a group number renewed, a dependent aged out. Verify coverage before the visit, every visit. It's the cheapest denial prevention you have.

Heads up: CMS has proposed cutting payment 50% on the lower-valued service when an E/M and a procedure happen the same day (modifier 25). The final 2027 fee schedule rule is expected around November 1. If your providers routinely bill E/Ms with procedures, model the impact now so January doesn't surprise you.
