Is bariatric surgery safe?

Modern bariatric surgery has a low upfront risk, but safety is not limited to the day of surgery. The procedure selected can affect reflux, nutrition, monitoring, and other long-term considerations.

For example, the 10-year SLEEVEPASS study reported GERD in 31.4% of sleeve patients compared with 6.7% of gastric bypass patients. Individual risk still varies.

Before choosing a procedure, understand:

- The risks during and shortly after surgery
- How sleeve and gastric bypass may affect you differently
- What nutrition, supplements, and monitoring may be needed afterward
- How your symptoms, health history, and goals influence the decision

Gastric bypass can be a safe and effective option for selected patients and may improve reflux, but it also has its own long-term considerations. Sleeve surgery may be appropriate for others.

The goal is not to choose the same operation for every patient. The goal is to match the operation to the individual patient.

It was great getting to share the stage with fellow surgeons pushing the field forward — talking robotics, technique, and the future of patient care never gets old.

Let's set the record straight. There’s always some risk involved when you’re under anesthesia – that's true of ANY surgery. Bariatric surgery has come a long way and has an outstanding safety record, with serious complications being uncommon. Advances in technique and technology have made these procedures safer and more effective than ever before.

For many patients, the long-term health risks of severe obesity — including heart disease, diabetes and other chronic conditions — can outweigh the risks associated with bariatric surgery.

🇪🇦 ¿Es “peligrosa” la cirugía bariátrica? 🤔

Vamos a dejar las cosas claras. Siempre hay cierto riesgo cuando te sometes a anestesia; eso es así en CUALQUIER cirugía. Excited to be heading to India for three days dedicated to the future of robotic bariatric surgery! 🇮🇳

I’m honored to serve as Course Director at IRCAD India’s Robotic Bariatric Surgery Course, alongside an outstanding international faculty.

📅 October 7–9, 2026

This course is about much more than learning how to operate a robot. It’s about developing the judgment, technique, and workflow necessary to use robotic technology effectively in modern metabolic and bariatric surgery.

Surgeons will have the opportunity for hands-on training across multiple robotic platforms, while discussing operative strategy, technical pearls, challenging anatomy, complications, and how robotics is changing the way we approach bariatric surgery.

One of the aspects I’m most excited about is the multi-platform experience. The future of robotic surgery will not be defined by a single robot—it will be defined by surgeons who understand how to adapt their skills across technologies.

A sincere thank you to IRCAD India and Dr. Mohit Bhandari for the invitation and opportunity to help lead this program, and I’m looking forward to teaching alongside Dr. Raj Palaniappan and the entire faculty.

Still deciding between the shot and surgery?

Most comparisons ask which one is riskier. That’s the wrong question.

Both have complications. What differs is when they show up.

Surgery front-loads it: one procedure, then it’s largely behind you — aside from the long-term monitoring specific to your anatomy.

A GLP-1 doesn’t work that way. There’s no single risky moment. Just continuous exposure, for as long as you’re taking it. A 2026 analysis of nearly 100,000 patients found the strongest GLP-1s carried close to three times the GI side-effect risk, with nearly twice the treatment dropouts.

None of that makes one option the winner. It makes them two different decisions.

So the honest question isn’t which treatment has complications. It’s which risk you’re willing to live with — weeks around one procedure, or years of ongoing exposure.

That’s not something to settle from a reel or a DM. Results vary, and the right path depends on your history, your BMI, and what you’ve already tried.

GLP-1 medications have changed the treatment conversation—but “safe” does not mean “risk-free.”

Most side effects are gastrointestinal, including nausea, vomiting, diarrhea, and constipation. The longer-term discussion can also include gallbladder risk, lean-mass considerations, and the reality that treatment often needs to be continued to maintain results.

The goal is not to discourage treatment. GLP-1 medications can be effective options for the right patient.

But every treatment choice should include an honest conversation about benefits, risks, follow-up, and what is sustainable over time.
